AFFIDAVIT OF LOSS/DESTRUCTION
Of Owner’s Duplicate Copy
(Transfer Certificate of Title No. 5564)

I, JOSE A. BALDOZA, of legal age, Filipino, married to Racquel Tandog Baldoza, and a
resident of Villa Sorabella Subd., Concepcion Grande, Naga City, after having sworn
in accordance with law, hereby depose and state; that –

1. I am the registered owner of a parcel of land covered by Transfer Certificate

of Title No. 5564 issued in my name and located at Villa Sorabella Subd.,
Concepcion Grande, Naga City;

2. As the registered owner, I was given an owner’s duplicate copy of the said
title by the Register of Deeds of Naga City;

3. However, in 2010, I found out that my copy thereof, which I kept in the attic
of our residential home, was already totally destroyed by termites;

4. Hence, my owner’s duplicate copy of TCT No. 5564 is now destroyed; A copy
of the certified true copy of the said title is herewith attached as Annex-A;

5. I have no outstanding encumbrances involving the property covered by TCT

No. 5564;

6. I will file the necessary petition in court for the issuance of a new owner’s
duplicate copy of TCT No. 5564; and

7. I am executing this affidavit to attest to the truth and veracity of the

foregoing based on my personal knowledge and on authentic records.
