MOTION TO AMEND DECREE OF ADOPTION

The PETITIONERS, by themselves, unto this Honorable Court, most respectfully state:

1. That they received a copy of the Decree of Adoption dated 11 June

2009;
2. That they noticed some manifest errors therein, to wit:

In the fourth paragraph of the said Decree, it is stated

that the name of one of the petitioners is WILLIAM GORDON
SCANLAN instead of WILLIAM EDWARD SCANLAN, which is the
true and correct name of the petitioner

In paragraph five of the said Decree, the name of one of

the children of petitioner William Edward Scanlan was
erroneously stated as WILLIAM EDWARD SCANLAN instead of
WILLIAM GORDON SCANLAN which is the true and correct name
of one of the children of the said petitioner;

3. That in order to avoid future confusion in the records of the adopted, it

is necessary to make the above-mentioned corrections in order to reflect
the true facts.

WHEREFORE, it is respectfully prayed of this most Honorable Court to amend the

Decree of Adoption by correcting the manifest errors as above-stated.
