    Description of Near Miss:
    Site of incident: - Battery 01 C/S draught area near between inclined
    manometer and PT.
    During “B” shift, Mr. Parvez Ramteke after reversing he went to check the tie rod
    of coke side in Batt#01 on the way of returning his lower side of trouser tag with the
    corner of the tool and he fell down.

    Remedial measures suggested:


    1. The both tools must be replaced by new one single tool and its corner must
    be in round shaped.
    2. The technicians were advised to ensure six directional sight.
