prepared study map.If you attended the said forum and have not affixed your signatures, see the Cluster Head ASAP on Saturday, 3pm to
6pm, and bring the required documents like photocopy of accredited subjects, etc)

Dear Miss Jumao-as:

This is to authorize MS. GERRA MAE VILLARIN to pre-enlist the following subjects reflected on my study
map:

Ac16, Math 30, Mark 25, Mark 31, Mark 35

Purpose/s of this authorization is/are: I HAVE ALREADY BOOKED A FLIGHT GOING TO KOREA
prior the pre-enlistment announcement.

With this authorization,

*I am aware that I will only enroll subjects advised (reflected on the map). Should there be changes because of
failing grades this semester, it is my responsibility to request for a revised study map, which is the basis of my
pre-enlistment.

*I am authorizing the same person to PROCESS PETITION OR TUTORIAL for off-sem/ off year subjects; on
sem subjects but schedule/section has less than minimum number of enrolless/ pre-enlistees.

*I have attached valid proof (photocopy of plane ticket).
