JOB DESCRIPTION

POST TITLE: Community Staff Nurse


GRADE: Band 5
HOURS: 1.0 WTE – 37.5 hours /week
LOCATION: CCNT team
REPORTING TO: Band 6 CCN
ACCOUNTABLE TO: Service Manager CCNT

Job Purpose:

 Work within the established Community Children’s Nursing Team providing evidence based
nursing care for in the community setting under the supervision of the Children’s
Community Nursing Sisters.

 In conjunction with the case holder caring for specific children and their families in the home
or other community setting, including liaising/organising with partner agencies and
attending multi-agency care planning meetings.

Main Duties and Responsibilities

1. To manage a defined caseload of clinical work under the supervision of the Caseload
holder

2. To work as an effective team member

3. To undertake comprehensive nursing assessments

4. To provide theoretical training and competency based assessments in relation to nursing


interventions to partner agencies

5. Work with all relevant staff and parents involved in the child/young person’s care

6. Ensure a robust health care plan is developed reflecting the needs of the child or young
person

7. Update health care plans and ensure these are clearly documented and understood by all
staff

8. Contribute to the written (System one) records detailing any specialist health
support/advice within day to day recording

9. To develop special interests and links in specific areas of Children’s Community Nursing,
for example tissue viability, paediatric oncology, in order to keep up with current research
and update other members of the team

10. The post holder must at all times carry out his/her responsibilities with due regard to the

Communication

1. To promote inter-agency working and joint assessment (Health, Social Services, Education
& Voluntary Sector) in order to utilise all available resources in the provision of seamless
care.

2. Prepare reports as required for reviews

3. The post holder will communicate effectively with key stakeholders, including clients,
statutory and non-statutory organisations

Human Resources

Maintaining own professional development and requirement to take part in appraisal and KSF
process

Research & Development (R&D) Activity

1. To assist in developing specific evidence based areas of Children's Community Nursing


within the service, according to client / service needs in order to enhance care provision
and make best use of resources

2. Required to undertake R&D activity

Clinical and Practice Governance

1. Observe and maintain strict confidentiality with regards to any patient/family/staff/records


and information in line with the requirements of the Data Protection Act.

2. Any data that is taken/shared as part of a phone call or transported, transferred


electronically must be undertaken with regard to the Trust Information Governance and
Information Security policies.

3. The post holder must adhere to the Trust risk assessment and risk management processes

4. The post holder must adhere to infection control policies and procedures

5. It is a condition of your employment that you are currently registered with NMC it is your
responsibility to maintain your professional registration.

6. Undertake mandatory training and any other training relevant to the role as required by
Cambridgeshire Community Services NHS Trust.

7. The post holder must participate in clinical and other audits as required

8. Provide patients and their families/carers with information on standards they should expect
from the team

9. Participate in Restorative/clinical/safeguarding supervision on a regular basis

10. The post holder is required to participate in relevant emergency preparedness process for
their team.
