THIS IS TO REPORT THAT, we the undersigned PT.MARGO INDONESIA INTERSERVICE as a member
Superintendent of FOSFA International, on behalf of:
CONTROL UNION
have attended prior to, during and after loading, have sampled, as well as conducted the inspection for
cleanliness, dryness and tightness and have determined the quantity of the parcel referred to above shipped
on board captioned vessel and herewith submit our findings as follows :
WEIGHT
The cargo was delivered via shorelines connected with flexible hose connected to manifold into vessel’s tank
no(s). 7P and 8P.
We hereby declare that under our constant supervision, the delivered weights are as follows:
TOTAL QUANTITY SHIPPED WEIGHT: 499.942METRIC TONS
As reported by the supplier concerned the following were distribution of weights:

Tank Coating
The vessel’s tank no (s).7P and 8P areSTAINLESS STEELand as far as could be seen did appear in sound
condition without excessive loose scale and/or blisters.
LAST THREE CARGOES:
As declared by the vessel’s officer, the following previous cargoes did not contain any leaded petroleum or other
leaded products on her last three cargoes carried in the tanks.

Therefore on the basis of declaration made by the vessel at the time of loading the last cargo in each of the
relative ship’s tank were a substance appearing on the FOSFA International List of Acceptable Previous
Cargoes in force at the date of Bill of Lading and not on the FOSFA International List of Banned Previous
Cargoes currently in force.

PRESHIPMENT SAMPLING AND SAMPLING ON BOARD THE SHIP:


Pre-shipment shore tank samples before loading and vessel’s tank samples were drawn in accordance with the
FOSFA International/PORAM/PORIM Process Palm Oil Storage, Transportation, Sampling and Survey guide
ISO 5555.

Eight (8) representative samples were drawn from each shore tank before loading (the nearest practicable point
from vessel’s rail).
Eight (8) fully representative samples were drawn from each of the loaded vessel’s cargo tank of which two (2)
these samples were handed to the vessel’s Master with instruction to hand them over to the receiver at the port
of discharge and one (1) each of these samples submitted to an independent certified analyst licensed by the
Palm Oil Registration and licensing Authority.
The pre-shipment (shore tank) samples as well as further remaining samples retained by us for a period of three
months from date of bill of lading to ensure that these samples be available on demand to any receiver in the
event of a contamination claim.
