ACCOMPLISHMENT REPORT
GRADE 10 CURRICULUM
SY 2017-2018

I. IINTRODUCTION
Monitoring and tracking the records of the curriculum are the most important duties of the curriculum
chairpersons. Enrolment, sectioning, and the choices of TLE or minor courses (for Grades 9 and 10 only) are
just the first things that every curriculum chairman has to do before the school year starts.
Interviewing and asking some important questions are asked in every enrollee, especially to the
transferees, dropped outs, retained, and balik-aral students. All throughout the year, curriculum
chairpersons are always asked to give and immediately disseminate information, not only for the students,
but also to their respective advisers that subordinates them.
School forms (SF1, SF2, SF3, and SF5); form 137; LIS (Learner Information System); quarterly recognition
and providing certificates; PTA meetings, attendance and documentation; BoSY and EoSY; engaging with the
school guidance counselor, EBEIS, and SBM coordinators, especially to the head of the school; and school
and student activities are the things to be monitored and kept for the whole school year.

I. CURRICULUM CHAIRPERSONS
The following names are the chairpersons for the school year 2017-2018:

II. LIST OF ACTIVITIES

A. Enrolment:
The enrolment had started from May 15-June 2, 2017.
Every student/enrollee has to accomplish the requirements listed below:
1. Enrolment form (to be filled up by the parent or guardian)
2. Report card
3. Good Moral Certificate
4. Photocopies of the following:
a. Birth Certificate (issued by the PSA or NSO)
b. Cedula or Barangay Certificate
5. Long brown envelop

B. First PTA (Parents-Teachers Association) General Orientation

The first PTA General Orientation was conducted last May 31, 2017 (Wednesday).

The parents were oriented on the ff.

1. Rules and regulations of the school

2. Class schedules
3. Introduced Grade level Advisers and teachers.
 C. PARENTING SEMINAR AND ELECTION OF GPTA OFFICERS

This was conducted last July 11, 2017. The main objectives of the meeting were to identify the PTA
projects and the list of classroom officers for the election of the Mother or GPTA (General Parents-
Teachers Association) Officers.

D. NAT (National Achievement Test)

The NAT was given last October 3, 2017, from 6:30 am up to 3:30 pm to all the officially enrolled Grade
7 students wherein it was headed by the Grade 7 Curriculum Chairman Ma’am Maria C. Lauden, together
with the school Testing Coordinator and his Assistant Sir Luisito Jaoquin and Sir Randy Lojo.

E. NATIONAL CAREER ASSESSMENT EXAMINATION (NCAE)

The assessment was given last September 5- 6, 2017, from 6:30 am up to 12:00 pm to all officially
enrolled Grade 9 students. The preparation was headed by the Grade 9 Curriculum Chairman Ma’am
Celeste V. Bantog.

F. INTRAMURAL 2017

The event was held last September 28-30, 2017. It was conducted by the MAPEH Coordinator Ma’am Elsa P.
Sabanal.

G. TEACHER’S DAY
A post celebration of the World Teachers’ Day had happened in San Jose del Monte National High
School last October 9, 2017. We, the curriculum Chairman awarded some teachers on their performance
as adviser.
The following are some of the awards.

2. Zero Dropped Out and 100% promotion ( M’ Brenda D. Adriano, M’ Amalia B. Fernandez, Sir
Mark Gil Nicolas, Sir Emmanuel I. Espiritu, M’ Ma. Ellaine L. Quintos.
3. Efficient Teacher/Adviser ( M’Amalia B. Fernandez, M’ Emeeluz G. Arante.
4. Best in Room Structuring ( SPA – B, M’ Emeeluz G. Arante, Magsaysay , Maria Charina A. Jaen)
5. Champion Coach in Intramural 2017 ( Sir Voltaire Amor, Badminton, Maria Charina A. Jaen,
Table Tennis, Helen Grace Pulan, Chess, Jovella Villablanca, Volleyball Boys, Sir Randell O.
Cabales, Athletics)

H. Moving Up Ceremony
The most awaited ceremony of the Grade 10 students witness the receiving of
completers’ certificates as they come up the stage last April 5, 2018 with a theme of
“ Mag-aaral ng K to 12 Handa sa Hamon ng Buhay.”.

 III. CHALLENGES ENCOUNTERED

The usual problems that the curriculum has been encountering are the following:

A. Enrolment
a. Late enrollees
b. Transferred-in students, especially from the private school who have no credentials
c. Continuous cancellation of the enrolment of some students due to:
c.1. no appearance/ no show
c.2. personal reasons that lead to transfer or pull out (Family problems/ change of residences)

B. PTA Meeting
a. Only few parents had attended the meeting (no one got a perfect attendance in any sections)
b. Deadline of payment for the agreed amount for the PTA projects for most sections was not
materialized.
IV. SOLUTIONS MADE

A. Enrolment
a. Late enrollees with due reasons were still accepted.
b. The parents were called and were given an extension for the deadline of submission of the
credentials (until the end of October).
c. Inevitable cases were accepted like change of residences.
d. SARDOS (Students At Risk of Dropped Outs) were called attention by informing their parents thru
home visitations done by the advisers and curriculum chairman in Grade 7.
e. Advisers were also advised not to enroll SARDOS in LIS (Learners Information System) to avoid
dropped outs.
B. PTA Meeting
a. Letters for the PTA Meeting were given to the parents to formally invite them.
b. Date of the meeting was done to the parents’ most convenient time.
C. NAT/NCAE
a. The slot of the student that had taken already the NAT/NCAE was given to other student who was
not in the list.
b. Students who were not attending classes regularly were not also included in the list of takers.
c. Continuous campaign of the importance of taking the NAT/NCAE .

V. OTHER PROBLEMS ENCOUNTERED

A. Littering

SOLUTIONS MADE
A. The problem was solved through monitoring of classroom cleanliness.
B. The teachers conduct a home visitation to some students who are no longer attending their classes.
C. There are some students reported to the guidance office that they were member of the fraternity. The students
that involved in the said group were decided to transfer to other school.

VI. RECOMMENDATIONS/SUGGESTIONS

A. Teachers must call parents/guardians attention if a student is absent for 3 consecutive days and above.
B. The school must have enough budgets to finance the needs of the students who participated in the contest.
C. Teachers must conduct relevant and functional researches for improvement in any aspects.
D. The school must conduct an orientation to the parents so that they will also be aware on how to handle if their
child if it is being bullied and also how to handle family problems without affecting the interest of the children in
school.
