PROVINCIAL TRAINING CENTER AGUSAN DEL NORTE

CONTRACT OF SERVICE

KNOW ALL MEN BY THESE PRESENTS:

This Contract of Service is made and entered by and between the Provincial
Training Center – Agusan del Norte with office address at Government Center,
Brgy., Cabadbaran City, herein represented by FLORENCIO F. SUNICO, JR. ,
Regional Director, hereinafter referred to as the First Party;

and

MR. RICARDO P. AMIL of legal age, married and a resident of P-3, Brgy. 9,
Cabadbaran City, Agusan del Norte hereinafter referred to as the Second Party;

WITNESSETH

That the First Party hires the services of the Second Party as Watchman to
be assigned at the TESDA Provincial Training Center, Government Center, Brgy.,
Cabadbaran City

That the Second Party possesses the required education, experience, skills
and knowledge to perform the job described herein;

That the First Party urgently needs to hire the Second Party due to the
exigency of the service and there is no existing Office Staff that can perform the
work;

That due to budgetary constraints, the First Party would not be able to hire
said services under casual or contractual appointment;

That the Second Party agrees that aside from his salary, no additional
monetary and non-monetary benefits shall be provided by the First Party and that
no employee-employer relationship exists between the First and the Second Party;

That the Second Party hereby attests that he is not related within the third
civil degree of consanguinity or affinity of the hiring authority, and or affinity to the
hiring authority, and/or representative of the First Party, that he has not been
previously dismissed from the government service by reason of an administrative
offense, and that he has not reached the compulsory retirement age of sixty-five
(65). Furthermore, the services rendered herein are not considered, or will never be
credited as government service;

That the Second Party agrees to render the required services at the PTC
Building Facilities from October 1, 2018- December 31, 2018, unless otherwise
