BAC RESOLUTION CONSIDERING THE WAIVER LETTER

RESOLUTION NO. 2018 – 060 – A

WHEREAS, Bids and Awards Committee of the Local Government Unit of Siaton received a waiver letter dated March 7,
2018 from “UY CONSTRUCTION AND DEVELOPMENT” to supply and deliver various construction materials for
project to viz;

2016 – 447 Concreting of Barangay Road at Mantiquil, Siaton Negros Oriental
2016 – 449 Concreting of Beach Road – Bonawon of Bonawon, Siaton Negros Oriental
2016 – 550 Concreting of Barangay Road at Marait 2 Road, Cabangahan Siaton Negros Oriental
2016 – 554 Concreting of Barangay Road at Lamtok, Casalaan, Siaton Negros Oriental
2016 – 557 Concreting of Barangay Road at Barangay Bonbonon, Siaton Negros Oriental

WHEREAS, considering that “UY CONSTRUCTION AND DEVELOPMENT” cannot proceed in the delivery of
construction materials due to price increase and considering the time elapsed in serving the Purchase Order (P.O);

WHEREAS, upon deliberation and in response to the letter of “UY CONSTRUCTION AND DEVELOPMENT”, the
committee unanimously agreed to grant on the request and return the Performance Bond accordingly;

NOW, THERFORE, We the members of the Bids and Awards Committee, HEREBY resolve as it is RESOLVED to
recommend for approval by the Municipal Mayor of the Municipality of Siaton, Negros Oriental the following findings.

RESOLVED, at the Municipality of Siaton this 26th day of March 2018.
