Re: Early Maternity Leave

Dear Ma’am,

I am Aireene Ley E. Lacanaria and I am working as a company teller at Iligan Light and
Power Inc., under the supervision of RC barretto Gen. Services. I am writing to confirm
that I am pregnant and that my baby is due in the first week of October, 2019.
Therefore, I am applying for my Early Maternity Leave starting august 2019 and I shall
be able to resume duty on a total of 105 days Maternity leave, and I wish to avail my full
maternity leave along with my annual leave allowance provided by Social Security
System.

I will make sure that you will be informed about any changes in the plans and I will also
remain in contact with my Treasury management supervisor of Iligan Light Mr. Geoffrey
Baclayon and my RC Barretto Gen. Services head teller supervisor Mr. Kevin Q.
Cabatingan and assistant head teller Mr. Bryan E. tutor from time to time.

I hope you will process my application and approved my Early maternity leave as
possible.

Attached here is the Copy of my Ultrasound result and the doctor recommendation for
admission due to Preterm Labour, including a one month rest.
