TRAINING CONTRACT

I, , BS Civil Engineering student of Marinduque State

College and enrolled in the Practicum/Internship/On-the-Job Training Program as an academic
requirement of my course, do hereby enter into a contract with Marinduque State College (MSC) as
Higher Education Institution (HEI) and the _____________________________________ as Host
Training Establishment (HTE) with the following terms and condition.

1. That I shall comply with all the requirements and policies of the Higher Education Institution
in implementing a Practicum/Internship/OJT Subject
2. That I shall abide with the rules and regulations to be imposed by the

3. That I shall exercise due care and diligence in performing tasks assigned to me as student
trainee.
4. That I shall renounce and waive my claim against the Higher Education Institution and
