GREEN= Low Priority “CONTINUE!”
Reviewed existing SOP’s, appropriate PPE, related procedures etc. before continuing with task.
REDUCE THE RISK: Following the initial Risk Rating (e.g. RED #1-Major/Very Likely), implement
Controls to Reduce or Eliminate the Hazards, thereby reducing this Risk Rating (e.g. GREEN #3 Minor/Very
Unlikely) by creating an SOP, train employees, erect warning signs, wear the correct PPE etc.

*Lockoff Hasp -multi-lock safety device used in conjunction with isolation


tags and/or when more than one person is working on equipment, enabling
workers to protect themselves from unexpected start-up of machinery or
electrical circuits whilst they carry our maintenance or cleaning work. The
hasp is attached to the primary isolator with a personal padlock during long
term works.

Shut Down
 Once works have been completed and only once plant deemed safe
for operation, the person(s) card(s) can be removed only by the
person(s) that applied them or by a Supervisor after consultation
with signatory of the tag.
