1. TIN of Estate

2. Photocopy of death certificate

3. A. Deed of Extrajudicial Settlement

B. Affidavit of Self-Adjudication
C. Court Order (if judicially settled)
D. Sworn declaration of all properties of Estate

4. O.R. / deposit slip (payment of tax)

5. Certified true copy of latest tax declaration of the lot and the
improvements

6. Owner’s copy of TCT

7. If applicable: sworn declaration of no improvement by at least 1 of the

transferees OR Certificate of No Improvement from Assessor’s Office
