    Subject: Request letter for using the venue

    Madam,

    Good day!

    The City Schools Division of Cabuyao will hold its annual City Athletic Meet on
    November 13 – 14 as stated in UM – 419 s. 2018 and UM No. 400 s. 2018 City Athletic
    Meet (Dual Meet). This activity aims to select athletes who will represent our division in the
    2018 Inter Division Sports Competition.

    In lieu with this, we are asking your good office to please allow us to use the following
    facilities for the said event with the corresponding schedules and venue. Hence, we will take
    full responsibility to any damages or loss that may take place during the event.

    November 13 -14 Badminton Court – PnC Gymnasium 4th floor 7AM – 6PM
    PnC Volleyball Court 7AM – 6PM

    Your favourable response regarding this request will be highly appreciated.
