History
CPH was discovered by Norwegians Ottar Sjaastad and Inge Dale in 1974. The term 'Chronic
paroxysmal hemicrania' was first used in 1976 by Sjaastad to describe a condition seen in two
of their patients who were experiencing repeated solitary and limited daily headache attacks
on only one side of the cranium.[2]
It is possible that Paroxysmal Hemicrania was first described by Johann Oppermann in 1747
under the term “hemicranias horologica”. Oppermann's report included a 35-year-old woman
who had hemicranial pain that lasted for 15 minutes and recurred regularly every hour.[3]
CPH has been included in the International Headache Society's classification system since
1988.[2]
Diagnosis
The temporal branch of the facial nerve is immediately above the eye.
CPH is a long-term disease with symptoms lasting for longer than a year, either without
remission or with remissions that last less than a month.[1] In order to be diagnosed with CPH,
a patient needs to have had at least 20 attacks filling the following criteria:
In addition, diagnosis of CPH requires that neuropathy of the supraorbital area in the
temporal branch of the facial nerve be ruled out.[5]
Epidemiology
Although CPH is often compared to cluster headaches, it is much less prevalent, occurring in
only 1-3% of those who experience cluster headaches. CPH occurs roughly in 1 in 50,000
people, while cluster headaches are comparatively more common and are found in 1 in 1000
people.[1] Cluster headaches occur primarily in men, while CPH is more commonly diagnosed
in women. [3] The female to male ratio of diagnosed patients can range anywhere from 1.6:1
to 2.36:1. [1] Symptoms may begin to appear at any age, but onset usually occurs in adulthood
with a mean starting age within the thirties. [4][6]
Symptoms
Individuals with CPH suffer multiple short, severe headaches a day, often more than five,
with most lasting between 5 and 30 minutes each. When compared to cluster headaches, CPH
attacks are typically shorter.[3] Each headache is centered around the eye, temple and
forehead and is localized to one side of the head. While redness and watering of the eye are
associated with CPH, patients typically do not experience nausea or vomiting.[7]
Treatments
A ten-patient study conducted by Pareja et al. found that all patients diagnosed with CPH
were responsive to indometacin and were able to completely control their symptoms. Doses
of the drug ranged from 25 mg per day to 150 mg per day with a median dose of 75 mg per
24-hour period.[8] Almost all cases of CPH respond positively and effectively to indometacin,
but as much as 25 percent of patients discontinued use of the drug due to adverse side effects,
namely complications in the gastrointestinal tract.[9] According to a case study by Milanlioglu
et al., 100mg of lamotrigine, an antiepileptic drug, administered twice daily alleviated all
painful symptoms. No side effects were noted after two months of treatment. Dosage of
lamotrigine was decreased to 50mg a day after the first two months, and no symptoms or
side-effects were recorded after a three-month followup.[10]
Use of topiramate has also been found to be an effective treatment for CPH, but cluster
headache medications have been found to have little effect.[5]
Different gender distributions - CPH is more common in women than men, with
opposite occurring with cluster headaches.
CPH attacks occur more frequently, but are shorter.
Individuals with CPH are far more responsive to Indomethacin than individuals with
cluster headaches.
CPH attacks can be provoked by neck movement.
In a study conducted by Sjaastad, heating a patient's body will cause the painful side
of the forehead to sweat more in CPH patients, while there will be less sweating on
that side for those suffering from cluster headaches.[3]

Possible Secondary Causes Many secondary conditions have been reported to be
possible causes of CPH, according to Mehta et al., most of which are arterial
abrasions or tumors. These include aneurysms in the circle of Willis, middle cerebral
artery infarction, parietal arteriovenous malformation, cavernous sinus and petrous
ridge meningiomas, pituitary adenoma, Pancoast tumor, gangliocytoma of the sella
turcica, and malignant frontal tumors.[11] This accentuates the urgency for those
diagnosed with CPH to receive an MRI head scan. Comparison to Cluster Headaches
Though outwardly similar to cluster headaches, Chronic Paroxysmal Hemicrania is
rather different, and the two headaches are not a subset of one or the other. Key

Hemicrania kronis paroksismal (CPH), juga dikenal sebagai sindrom Sjaastad, adalah
melemahkan sakit kepala unilateral parah biasanya mempengaruhi daerah sekitar
mata. Biasanya terdiri dari beberapa parah, namun pendek, serangan sakit kepala yang

Sejarah
CPH ditemukan oleh Norwegia Ottar Sjaastad dan Inge Dale pada tahun 1974. Istilah
'paroxysmal hemicrania kronis' pertama kali digunakan pada tahun 1976 oleh Sjaastad
untuk menggambarkan kondisi yang terlihat pada dua pasien mereka yang mengalami
berulang soliter dan terbatas serangan sakit kepala harian hanya pada satu [2] sisi
tempurung kepala.
Ada kemungkinan bahwa Paroxysmal Hemicrania pertama kali dijelaskan oleh

Chefalgia / sakit kepala ialah salah satu keluhan fisik paling utama manusia. Sakit kepala
pada kenyataannya ialah gejala-gejala bukan penyakit & bisa menunjukkan penyakit organik
(neurologi / penyakit lain), respon stress, vasodilatasi (migren), tegangan otot rangka (sakit
kepala tegang) / kombinasi respon tersebut (Brunner & Suddart).
Klasifikasi sakit kepala yg paling baru dikeluarkan oleh Headache Classification Cimitte of
the International Headache Society sebagai berikut:

Manifestasi Klinis
1. Migren
Gejala & gejala-gejala adanya migren pada serebral merupakan hasil dari tingkat
iskhemia kortikal yg bervariasi. Serangan dimulai dgn vasokonstriksi arteri kulit
kepala
dam pembuluh darah retina & serebral. Pembuluh darah intra & ekstrakranial
mengalami dilatasi, yg menyebabkan nyeri & ketidaknyamanan.
o Fase aura.
Periode aura ini berhubungan dgn vasokonstriksi tiada nyeri yg diawali dgn
perubahan fisiologi awal. Aliran darah serebral berkurang, dgn kehilangan
autoregulasi laanjut & kerusakan responsivitas CO2.
Fase sakit kepala berdenyut yg berat & menjadikan tak mampu yg dihungkan
dgn fotofobia, mual & muntah. Durasi kondisi ini bervariasi,
beberapa jam dlm satu hari / beberapa hari.
o Fase pemulihan
2. Cluster Headache
Cluster Headache ialah beentuk sakit kepal vaskuler lainnya yg sering terjadi pada
pria. Serangan datang dlm bentuk yg menumpuk / berkelompok, dgn nyeri yg
menyiksa didaerah mata & menyebar kedaerah wajah & temporal. Nyeri diikuti mata
berair & sumbatan hidung. Serangan berakhir dari 15 menit hingga 2 jam yg menguat
& menurun kekuatannya.
Tipe sakit kepala ini dikaitkan dgn dilatasi didaerah & sekitar arteri ekstrakranualis,
yg ditimbulkan oleh alkohol, nitrit, vasodilator & histamin. Sakit kepala ini berespon
terhadap klorpromazin.
3. Tension Headache
Stress fisik & emosional bisa menyebabkan kontraksi pada otot-otot leher & kulit
kepala, yg menyebabkan sakit kepala oleh tegang.
Karakteristik dari sakit kepala ini perasaan ada tekanan pada dahi, pelipis, / belakang
leher. Hal ini sering tergambar sebagai “beban berat yg menutupi kepala”.
Sakit kepala ini cenderung kronik daripada berat. Pasien membutuhkan ketenangan
hati, & biasanya kondisi ini merupakan ketakutan yg tak terucapkan. Bantuan
simtomatik mungkin diberikan untuk memanaskan pada lokasi, memijat, analgetik,
antidepresan & obat relaksan otot.

daerah wajah. IHS tahun 1988 menyatakan bahwa nyeri pada wajah termasuk juga dalam
sakit kepala. Dalam buku2 teks dan jurnal banyak memakai klasifikasi 1962, dan klasifikasi
terbaru adalah INS 1988 yang akan dipakai dalam ICD-WHO ke-X ada beberapa terminologi
yang harus dibedakan seperti : Pusing = vertigo, ringan kepala= like headedness, pening =
dizziness, rasa ingin pingsan = faintness, kepala berdenyut tujuh keliling dan sebagainya.
Definisi menurut IASP (International assosiation for the study of pain): Nyeri adalah
pengalaman sensorik dan emosional yang tidak menyenangkan yang sedang terjadi atau telah
terjadi atau yang digambarkan dengan kerusakan jaringan Mekanisme nyeri : Nociseptor
yang diterima reseptor2 di kulit, pembuluh darah, visera, muskulusskeletal,dan lain-lain,
jalannya sebagai berikut : reseptor-- syaraf tepi -- medulla spinalis-- thalamus--korteks. Dari
sini baru ada reaksi emosi – psikis- motorik tanpa ada modulasi, sedangkan dalam perjalanan
hanya kesan sensorik. Batasan sekarang : nyeri adalah pengalaman subjektif, sangat pribadi
dipengaruhi oleh pendidikan, budaya, makna situasi dan kognitif ( menurut Bonica,Melzack).
Ada beberapa teori mengenai mekanisme nyeri kepala : - Teori Melzack & Wall (1985) : “
Teori gerbang nyeri “ bahwa : Nyeri diteruskan dari perifer melalui saraf kecil A delta dan C
rasa raba, mekanik dan termal melalui A delta A beta dan C ( serabut besar, kecepatan hantar
serabut besar lebih tinggi dari serabut kecil ). Disubstamtia Gelatinosa (SG) ada sel-sel
gerbang yang dapat bekerja menutup dan membuka sel T (targaet). Serabut besar aktif
merangsang sel gerbang di SG, sel gerbang aktif dan sel T tertutup, maka nyeri tidak dirasa.
Serabut kecil aktif, sel SG tidak aktif, dan sel T terbuka maka nyeri dirasa. Bila dirangsang
bersama-sama, misal antara rasa raba, mekanik,vibrasi,dll dengan rangsang nyeri maka nyeri
tidak dirasa (seperti pada teknik tens, DCS, koyo-koyo, dll.) Didapatkan kontrol desenden ke
medulla spinalis dari pusat2 supra spinal (emosi,pikiran, dll). - Konsep II: “Central Biasing
mekanism” Diduga ada daerah batang otak jadi ”CBM” yang menyebarkan impuls nyeri
keberbagai tempat diotak dan dapat menimbulkan inhibisi ke medulla spinalis. Ternyata
formatioreticularis peri-acuaductus dan peri-ventriculer kaya akan reseptor2 morpin dan
serotonin. - Konsep III ; Pembangkit pola Bila nyeri khronik telah membuat pola (gambar
diotak), yang dapat dicetuskan oleh input sensorik lain. Klasifikasi HIS 1988 ; 1. migrain 2.
nyeri kepala tension 3. nyeri kepala cluster dan hemicrania kronik paroksismal 4. nyeri
kepala yang tidak berhubungan lesi structural 5. nyeri kepala berhubungan dengan cedera
kepala 6. nyeri kepala berhubungan dengan gangguan vaskuler 7. nyeri kepala berhubungan
denagn gangguan intrakranial non vaskuler 8. nyeri kepala berhubungan dengan zat-zat atau

Jaringan2 yang menutupi tengkorak, scalp( kulit, jaringan kulit areolar,periosteum),otot-otot
kepala, mata, hidung, telinga, gigi geligi dll. B. Beberapa struktur intra cranial: a. saraf2
otak(N.cranial), b. saraf2 spinal: C1,C2 dan C3 c. arteri2 diotak dan duramater d. vena2 dan
sinus2 dipermukaan otak e. duramater didasar otak Struktur2 tidak peka terhadap nyeri
kepala : 1. Cranium 2. otak (hampir seluruhnya) 3. sebagian besar dura 4. pleksus khoroidalis
5. dinding ependym ventrikel Diduga hanya mekanisme perifer saja (nyeri perifer) sebagai
penyebab nyeri kepala, tetapi Raskin(1988) menemukan kemungkinan nyeri sentral dapat
sebagai penyebab nyeri kepala. Pemeriksaan fisik : Dilakukan lengkap : pemeriksaan umum,
internus dan neurologik. Pemeriksaan lokal kepala, nyeri tekan didaerah kepala, gerakan
kepala ke segala arah, palpasi arteri temporalis,spasme otot peri-cranial dan tengkuk, bruit
orbital dan temporal. Pemeriksaan Penunjang : Pemeriksaan penunjang yang dapat dilakukan
untuk membantu mendiagnosa nyeri kepala seperti : 1. Foto Rongten kepela 2. EEG 3. CT-
SCAN 4. Arteriografi, Brain Scan Nuklir 5. Pemeriksaan laboratorium(Tidak rutin atas
indikasi) 6. Pemeriksaaan psikologi (jarang dilakukan). Strategi penanggulangan nyeri : 1.
pengobatan proses dasar atau kelainan fisiologik spesifik (kausa etiologi,patogenesa), missal
antibiotik untuk infeksi, spasmolitik untuk kolik, ergot untuk migren, dll termasuk
pembedahan bila diperlukan. 2. pengobatan psikologik/psikiatrik dan atau psikotropik yang
bertujuan untuk : menolong penderita untuk menyesuaikan diri dengan stress akibat nyeri,
dan mengobati faktor2 [sikologik yang mnyebabkan atau mengkambuhkan nyeri.. 3. terapi
medikamentosa berupa analgetik untuk pengobatan simptomatik nyeri, apabila pengobatan
spesifik tidak ada atau kurang memadai 4. terapi2 dengan metoda fisik yang sifatnya
simptomatik apabila pengobatan 1,2, dan 3 kurang memadai atau dianggap gagal..
5.Hypnotherapy sebagai salah satu alternatif mujarab dalam penanganan migrain yg
disebabkan psikosomatis MIGRAIN Beberapa hal mengenai migrain, yaitu : - terjadi familial
80% sebelum 20 tahun - periodik 4-72 jam - unilateral, atau bilateral (kadang2) - berdenyut

Glossopharingeal neuralgia Postherpetik neuralgia 1.Cluster Headache - laki2 : wanita = 5 : 1
- nyeri unilateral - lokasi orbital, supraorbital,temporal - sakit sekali, tidak berdenyut - awitan
15 menit sampai 3 jam - ada periode bebas nyeri - gejala penyerta : hidung tersumbat, pilek,
conjugtiva merah, lakrimasi, miosis. - Patofisiologi ; adanya letupan paroxysmal
parasympatis, pembengkakan dinding a. carotis interna, dan pelepasan histamin. - Terapi ;
abortif : O2 inhalasi ¼ jam, ergot alkaloid, sumatripan dan pengobatan preventif : veramil,
ergot alkaloid, indomethazin (NSAID). 2.Khronik Paroxismal Headache - = cluster tapi
serangan lebih sebentar ( 2 - 45 menit) - serangan > sering - wanita, hampir sama
gangguannya - efektif/responsive terhadap NSAID (Indometazin) 3.Tension Type Headache -
berat seperti diikat, penuh - intensitas ringan – sedang - bilateral - tidak mual/muntah -
photofobia/phonofobia - pengobatan : acetaminopen / parasetamol, ASA, NSAID,
antidepresan untuk yang khronik, psikoterapi. 4.Temporalis arteritis - inflamotori diseases cf
cranial arteries - umur > 50 tahun - unilateral kadang2 bilateral - temporalis superficialis -
Pengobatan ; Prednison 5.Trigeminal neuralgia - umur pertengahan - paroxysmal seperti
distrum - N.V1-2 - Terjadi beberapa detik- menit - Di ikuti gerakan involunter - Di stimulasi
oleh : bicara, mengunyah, gosok gigi, cukur jenggot, gerakan wajah, lidah. - Etiologi :
simptomatik ; multiple sclerosis, Aneurisma A. basilaris. - Pengobatan ; terapi kausalnya
berikan antikonvulsan jika ada cetusan2 6.Glossopharingeal neuralgia - nyeri
intens/paroxysmal di fossa tonsilaris - disertai bradikardia, sinkop - N.IX, auricular N. X -
Terapi : anticonvulsan, operasi. 7.Post herpetic neuralgia - berhubungan dengan vesikel pada
kulit - Herpes zoster virus - Nyeri sperti terbakar, tertusuk atau hiperesthesia, allodynia =
diusap/diraba menimbulkan nyeri - Waktu infeksi akut Herpes zoster tidak nyeri - Terapi ;
antikonvulsan, anti depresan - Pencegahan dengan pemberian Acyclovir - Banyak terjadi
pada orang tua 8.Temporal arteritis - Inflamasi A. temporalis - Usia > 50 tahun - Berdenyut /
tidak berdenyut - Tajam/ ditusuk - Unilateral atau kadang2 bilateral - A. temporali
superficialis menonjol, jika diraba neri tapi tidak berdenyut,tidak enak badan, berat badan
turun,Demam, anemia, nyeri otot,BSE meningkat - Trombosis a. opthalmica, a.cilliaris
posterior menimbulkan kebutaan - Diagnosa biopsy - Terapi ; Prednison 9. Dengan
Hypnotherapy dan Relaxasi Terutama pada penderita gangguan yang diakibatkan oleh
