Dear Ma’am:

In line with the upcoming National Medical Admission Test on October 21, Sunday, we, a few selected
students of BMLS IV-A, would like to formally request an excuse from attending our duty and review
class on October 20-22 since the test will start at 7:00 in the morning and end by 4:00 in the afternoon
and the available boat trips are within the requested dates.

We understand that we will be responsible for our own welfare and fully acknowledge that the
College of Medical Technology and the school itself will not be held responsible for any untoward
incident that may arise during the said dates.

Please let us know whether you have granted an excuse, and if so, how we can arrange for us to make
up for our missed duties and classes. Thank you most sincerely for considering our request, and we
look forward to hearing from you soon.
