POLICIES, RULES, AND REGULATIONS

GOVERNING CLASSROOM AND LABORATORY INSTRUCTION


(NOTE: with Policies on Academic Consultation and Advising Highlighted)

1. ATTENDANCE
1.1 Absences
1.11 Each instructor during the lecture period shall maintain a seat plan for each class for
the monitoring of student’s attendance.
1.12 If the number of unexcused absences incurred by the student exceed twenty (20%)
percent of the total number of hours prescribed in the curriculum, then he/she will be
automatically dropped from the subject.
1.13 Two (2) LATES in the lecture will be equivalent to one (1) absence.

2. UNIFORMS
2.1 Students are required to wear a complete prescribed uniform in class and during
examinations.
2.2 Gentlemen
2.21 Prescribed white polo shirt and black slacks
2.22 Undershirt should be plain white
2.23 Black /dark colored socks and black closed leather shoes
2.24 Haircut should be one (1) inch above the collar of the polo shirt
2.25 No sporting of beard, moustache or goatee
2.26 No female accessories like earrings and headbands
2.3 Ladies
2.31 Length of uniform should cover the kneecap
2.32 Long hair should be neatly tied (ponytail/pigtail)
2.33 Black closed leather shoes (flat/pumps with one (1) inch heel)
2.34 Only white/skintone undergarments should be worn
2.4 Students are required to wear their school ID at all times.

3. SCHEDULE OF ACTIVITIES
3.1 Any changes or new activities of schedules will be posted on the bulletin board. Otherwise,
these are already noted on the Course Outline given to students at the start of the term.
3.2 Make up classes will be made for holidays and other school events that will take up lecture
hours. All make up classes are pre-announced.
3.3 For MTLBE: Make up classes will be on Saturdays.

4. LECTURE
4.1 Students are expected to read the assigned lecture topics before coming to class.
4.2 Students are encouraged to ask questions if they do not fully understand the lecture.

5. LABORATORY
5.1 Students are expected to read and study the assigned laboratory exercise before coming
to the laboratory.
5.2 Manuals
5.21 Drawings/results in the laboratory exercises will be signed by the preceptor.
Any unsigned laboratory exercise/rubric is considered as an absence in the laboratory
period and does not earn a credit for that particular exercise.
5.22 Manuals are collected after the Midterm Exam and after the Pre-Final/ Final exam for
grading.
Manuals submitted late will be given five (5) points deduction per day after the set deadline.
5.3 Attire
5.31 Laboratory gown, gloves, and surgical mask are required when laboratory exercises
deal with live microorganisms or when clinical specimens are utilized in an exercise.
5.32 The student will not be allowed to attend the laboratory class if he/she is not wearing
the prescribed protective proper attire.

6. EXAMINATIONS
6.1 All examinations and their respective venues will be posted.
6.2 Scheduled written examinations may utilize the Op-Scan answer sheets.
6.21 Each student will be charged with the number of Op-Scan answer sheets that will be
used per subject for the entire semester. This should be fully paid by Midterms at the
school cashier.

6.3 Types of Examinations:


6.31 LONG LECTURE/ LECTURE QUIZ: This will include topic/s within a designated
period before the scheduled exam.
6.32 MID-TERM EXAMINATION: This will cover the first chapter up to the topic which will
be announced by the examiner.
6.33 POST MID-TERM/ PRE-FINAL EXAMINATION: This starts with the topic right after
the Midterm exam coverage until the topic which will be announced by the examiner.
6.34 FINAL EXAMINATION: This will cover the first chapter up to the last chapter.
6.35 LONG LABORATORY EXAMINATION: The coverage will always be from the
beginning up to the exercise which will be announced by the examiner (true to
MICRO 1, PARA, and IMMUNO-SERO only).
6.36 LABORATORY QUIZ: This covers the exercise for the day and is given anytime during the
laboratory period (true to HScience, HCare, CC, Hema, AUBF, and IH only).
6.37 PRACTICAL EXAMINATION: The coverage will always be from the beginning up to
the exercise which will be announced by the examiner.
6.4 The type of examination will be at the discretion of the examiner.
6.5 Checking of the written examination will be done manually or with the use of the Optical Scanner -
machine processed answer sheet. Results of examination will be posted on the bulletin board
with the student’s ID number and the corresponding score.
6.6 Students are encouraged to complain for any error in checking or computation at the time when
the results are posted on the bulletin board. However, no complaints will be entertained two (2)
weeks after the examination results are posted.
6.7 Cheating will be dealt with severely. Any student caught cheating will be
recommended to the Dean for disciplinary action.

6.8 Special Examination


6.81 This is given only on the following conditions:
a. Illness – must present original medical certificate
b. Death of an immediate family member – must present a photocopy of the
death certificate
c. Legal matters – must present a copy of the required document
ALL THESE DOCUMENTS MUST BE PRESENTED TO THE DEAN TOGETHER
WITH AN EXCUSE LETTER SIGNED BY THE STUDENT.
6.82 This will be given to students who were not able to take the scheduled written
examination (based on 6.81), upon presentation of an excuse slip signed by the
Dean
within two (2) weeks after the student has reported back to class. IT IS THE
RESPONSIBILITY OF THE STUDENT TO APPROACH THE TEACHER IN-
CHARGE FOR THE MISSED EXAMINATION. The date and time of the special
examination will be scheduled by the teacher in- charge. The highest possible score in
a special exam is seventy five percent (75%).
6.83 A student forfeits the special examination if he/she fails to present the excuse slip
within
the specified period of time or if he/she fails to take the special examination during the
scheduled date.
6.84 In the event the student fails to take the special examination, then his/her score for a
scheduled examination will be “ten (10) points less than the lowest score” obtained in
the entire batch for that particular examination.
This scoring system also holds true for an UNEXCUSED ABSENCE in an
examination.
6.85 There will be no special examination for missed laboratory practical exam. However,
in case of serious illness, communicable diseases, death of an immediate family
member or an excuse in which the Department may deem reasonable, certain
considerations may be given to the student for the missed practical exam.
6.86 No special examination will be given to missed laboratory quiz (true to HScience,
HCare, CC, Hema, AUBF, and IH only) and return demonstration ( true to HCare).
6.87 No special examination will be given to missed short lecture quiz/ pop quiz.
6.88 Students who come in fifteen (15) minutes after an examination has started, even if no
student has submitted his/her test paper, that examination will be considered a special
examination for that student. (Refer to 6.82)

6.9 For Histopathology Laboratory ONLY


Bringing of Histopathology Slide boxes or mounted tissue sections outside the Velez College
laboratory is STRICTLY NOT ALLOWED. Loss of specimens, tissue blocks, or slide boxes is
NOT considered for a make-up laboratory processing.

7. GRADING SYTEM (FOR MTLBE ONLY)


7.1 The passing grade in all examinations is seventy (70) percent.
7.2 The Grading system:

***FINAL LECTURE GRADE = Midterm Grade (40%) + Post-Midterm Grade (60%)***

7.31 “NG” (no grade) will be given if the student fails to:
7.311 take the final written examination, or
7.312 take the final practical examination, or
7.313 submit the laboratory manual /unfinished project after the final examination, or
7.314 present a FINAL Admission Slip
NOTE: Upon completion of the written or practical examination, the highest possible score
in that test is seventy five percent (75%).
a. For other missed examinations, the ruling in 6.8 will be followed.

2. COMPLAINTS REGARDING THE FINAL GRADE


a. Grades submitted to the Dean’s office after the Board of Promotion’s meeting will
be deemed final.
b. Request for re-computation of grades may be made through the Dean’s office.
c. Changes in the grade will only be made if there is error after re-computation and
after this has passed through the board of promotions or through the Dean’s office.

3. COUNSELING OF STUDENTS
a. A student with poor academic performance or tardiness in the class shall be called
by the faculty or instructor and be referred to the class Adviser for counselling.
b. Any student with behavioural problems will be counselled by the class adviser
during his/her consultation hours. If the case cannot be settled by the class
adviser, he/she will endorse the student/s to the Dean for consultation before the
case will be referred to the Guidance office or Disciplinary Committee.

10. OTHERS
10.1 All electronic gadgets; i.e., cellular phones, iPODS, tablets, digital/mini-cameras (e.g., ballpens/
watches/ eyeglasses), and the like are STRICTLY NOT ALLOWED during lectures and during
examinations.
If caught using the unit during lectures, the item will be confiscated and returned at the end of the
day
after submission of an explanation letter signed by the Dean.
In the event a student is caught bringing/using the gadget during examinations, the item will be
confiscated and will be returned after the end of the term with clearance from the Guidance Office.

BRINGING OF ANY ELECTRONIC GADGET DURING EXAMINATION IS CONSIDERED A


FORM OF ACADEMIC DISHONESTY. This will be channelled through the Guidance Office.

10.2 Playing/Using game cards or mobile gaming devices during classes (lec and lab) will be
referred to the class adviser for investigation and counselling. If the case cannot be resolved
within the adviser’s level, it will be endorsed to the Dean.

11. Other rules and regulations stipulated in the Velez College students’ manual applies.

I hereby attest to have been explained about the course outline of the subject, MTBLE. I have been
made aware that passing the subject is necessary to be eligible to enroll the subsequent subjects in
the next semester.

I have been given a copy of the course outline of MTBLE including its grading system. I have also
been oriented of its requirements and commit to passing such requirements on time. I have had the
opportunity to ask questions during the orientation and any questions that I have asked have been
answered by my instructor to my satisfaction.
