ABSTRACT
Background
Prevalence of Aspergilloma in patients with cavitary or non-cavitary bronchogenic
carcinoma was rare. Only a few cases of aspergilloma combination with lung
cancer have been reported in literature. This case emphasizes the importance
of biopsy and cultures as fungal infection and Squamous Cell Carcinoma often
have similar, non specific clinical appeareances.
Case report
A woman 43 years old with chief complaint hemoptysis since 3 days ago,
hemoptysis. History of recurrent hemoptysis since 2 years ago with volume
around 300 ml/day. No shortness of breath, chest pain when coughing or when
take a breath. Loss of appetite and weightloss about 10 kg in last 3 months.
Laboratory data revealed decrease hemoglobin and HIV nonreactive. Acid Fast
Bacilli (AFB) stain was negative. Chest X- Ray showed suspected left lung mass,
CT-scan result Aspergilloma appereance in superior lobe of left lung and
Cytology Bronchial wash show results of Supurative inflamation lesion with
Aspergilloma. The patient was performed lobectomy on upper left lobe and the
result of histopathology showed Squamous Cell Carcinoma.
Discussion
We report our experience of the diagnostic in this patient with disseminated
squamous cell carcinoma with aspergilloma. Squamous cell carcinoma with
aspergilloma remains rare. The symptoms (such as malaise, weight loss, cough,
and hemoptysis) are non-specific and are overlapping those of a pulmonary
neoplasm. Pulmonary Aspergilloma or intracavity fungus ball is commonly found
in cavities in case of sequalae tuberculosis, but rare in cavitated squamous cell
lung cancer. Every case of lung carcinoma should be extensively searched for
aspergilloma and all cases of aspergilloma should be meticulously screened for
malignancy.
