I, ALEXANDER M. STA. MARIA, Filipino, of legal age, married, and a resident of

12 Juan Luna St., Fairview, Quezon City, after having been duly sworn in accordance
with law, hereby depose and say:

1. That I am the uncle of EDWARD SANTOS, a BS Biology Student at University

of Baguio, who is applying as a medical student at the Pamantasan ng Lungsod
ng Maynila for the school year 2019-2020;

2. That, because he has lost his father, NOEL B. SANTOS, and her mother, GINA
P. STA. MARIA, is unemployed, I have been supporting his studies;

3. That, I intend to continue supporting him financially as he pursues his medical

course at the PLM;

4. That, as Department Manager of the Project Management Department of

Golden Keys Corporation, I am gainfully employed and thus, financially capable
to pay for his academic and related expenses while at PLM. Enclosed is a copy
of my Individual Tax Return for 2017;

5. That, I am executing this affidavit in compliance with the requirements for the
medical school application of my nephew at PLM.
