MEMORANDUM

SUBJECT : Explanation re; Lost/Missing Administrative Case Folder with

docket MPDDLOS-1709-035

DATE : August 13, 2019

1. Reference: Memorandum from Chief, DLOS dated August 7, 2019 with

subject Notice to Report.

2. This pertains to the Lost/Missing Administrative Case Folder filed against

Pat Jason Bal-iyanDiaken and Pat Michael Salvador Gacusan for Simple Neglect of
Duty under Docket No. MPDDLOS-1709-035.

3. In this regard, the undersigned would like to inform that the said case
folder was lastly logout heading to the Office of District Personnel and Human
Resource Development Division, based on the logbook dated Month/Date/Year.
However, on Month/Date/Year it was return to the Office of Discipline Law and Order
Section for correction coming from the Office of Chief District Directorial Staff as
supported by logbook record dated Month/Date/.

4. As procedural practice of this Office, upon received of said case subject

for correction coming from other Offices, said case was personally turnover to the
drafter (Pat Limbo) concerned.

5. That the undersigned had no malice or intent to do such so. Hoping for
your kind consideration.
