AFFIDAVIT OF EXPLANATION

I, JELLINE L. ARCEGA , of legal age, Filipino, and resident of

#69 CLEOFER ST. STA. QUITERIA CALOOCAN CITY. , after having been
duly sworn to in accordance with law do hereby depose and state:

1.That I was candidate for graduation for the degree of Bachelor of Science
Hotel and Restaurant Management (BSHRM) course from the University of
Manila on May 2012.

2. That I was not able to claim my diploma and other scholastic documents
because I was busy looking for a job after graduation.

3.That I have submitted my Application for Candidacy for Graduation (ACG)

recently .

4. That I am executing this affidavit to attest to the truth and veracity of my

foregoing statement and for my intent to request f or my Special Order
( S.O. Number) from proper authorities concerned.
