AFFIDAVIT

I, EVA RALLESTAN y VIVAR, of legal age, Filipino

citizen, married and a resident of Blk 7 Lot Sec 15 Phase II,
Barangay Muzon, City of San Jose del Monte, Bulacan, after
having been duly sworn to in accordance with law, do hereby
depose and say, that:

1. I am the granddaughter of late FERMINA T.

SILVA;

2. Recently, my grandmother FERMINIA T. SILVA

died last June 17, 2016;

3. I am executing this affidavit to attest to the truth

of all the foregoing facts, to correct any discrepancy that
may arise therefrom and make the necessary adjustments.
