INSERTING IV CANNULA

PROCEDURE CD ID REMARKS
1. Verify the written Doctor’s order for IV therapy; check prepared IVF
and other materials needed.
2. Explain the procedure to reassure patient and significant others and
observe the 12 R’s.
3. Do hand hygiene before and after the procedure.
4. Choose site for IV insertion.
5. Dilate the vein.
Apply tourniquet 5 to 12 cm. (2-6 inches) above insertion site
depending on the condition of patient.
If vein is not sufficiently dilated:
a. Massage or stroke the vein distal to the site and in the direction
of venous flow toward the heart.
b. Encourage to clench or unclench the fist.
c. Lightly tap the vein with your fingertips.
6. Put on clean gloves and clean the venipuncture site.
Disinfect the site with antiseptic swab or cotton balls with alcohol in
circular motion and allow 30 seconds to dry (NO TOUCH
TECHNIQUE).
7. Insert the catheter
a. Use the non-dominant hand to pull or stretch the skin below the
entry site.
b. Hold the over-the-needle catheter at 15-30 degree angle with
bevel up, insert the skin through the skin and into the vein in one
thrust
c. Once blood appears in the lumen of the needle or you feel the
lack of resistance, reduce the angle of the catheter until it is
almost parallel to the skin, and advance the catheter until the
hub is 1mm to the puncture site.
d. Slip a sterile gauze under the hub
e. Release the tourniquet
f. Remove the stylet while applying digital pressure over the
catheter with one finger about 1-2 inches from the tip of the
inserted catheter
8. Connect the infusion tubing of the IVF to the IV catheter.
9. Anchor the cannula firmly with a transparent plaster or tape and a
small piece of sterile OS; apply splint if needed.
10. Discard all used materials observing infection control measures.
11. Document the procedure on the patient’s chart.
