1. Please download the application form.

2. Fill in the application form in hard format.

3. Submit your dues as per detail in the form in bank.

4. Take a photocopy of the application form.

5. Submit the original form in the concerned District Monitoring Officer (DMO) Office.

6. DMO will alot you username and password for the online submission of the same form.

7. Login to your account on Apply Online.

8. Submit the data in online software from the hard form.

Note: All those private schools who have already submitted their dues are requested to contact the
concerned DMO office for further registration process.
