Section 2: A Case History

This section will have the following structure and content:


1. Background Information
The first section of your paper will present your client's background. Include factors such as
age, gender, work, health status, family mental health history, family and social relationships,
drug and alcohol history, life difficulties, goals, and coping skills and weaknesses.

2. Family History:
(narrate the family history. Include the following information: parents’ and siblings’ names,
family address, parents’ job, birth order, description of the relationship with the parents and
other family members. If the client is married, include the name of the husband and children
if they have one. Describe also their relationship with each other. At least 5-8 paragraphs)

3. Social History:

(narrate the social life of the client in childhood, teenage years and adulthood. Put the
names of the client’s significant other, loved ones, friends, neighbors,
co-employees/workers, employers, teachers and more. Describe their relationship status. At
least 5-8 paragraphs)

4. Medical Health History:


(gather and narrate the information of the following. At least 1-5 paragraphs)

In the next section of your case study, you will describe the problem or symptoms that the
client presented with. Describe any physical, emotional, or sensory symptoms reported by
the client. Thoughts, feelings, and perceptions related to the symptoms should also be noted.
(At least 2-5 paragraphs)

10. Clinical Diagnosis (sample is the Case Analysis activity)


Provide your diagnosis and give the appropriate Diagnostic and Statistical Manual code.
Explain how you reached your diagnosis, how the client's symptoms fit the diagnostic criteria
for the disorder(s), or any possible difficulties in reaching a diagnosis.

Section 3: The Intervention

Treatment Planning

The second section of your paper will focus on the intervention used to help the client. You
are required to choose from a particular theoretical approach and counseling techniques
that are suitable for your client’s problem. Justify the chosen theoretical approach and
counseling techniques. (2 Paragraphs only)

4. Mental Status Examination:

character)

Instructions:

1. Choose a movie to watch with the group

2. No groups must have the same movie

3. Make a case study based on the chosen movie

4. Use the main character as your client

5. Client’s information on the case study will be based on the information shown in the
movie

6. Highlighted information (the color red) are optional. If the client does not have any of
those information needed, you may not include it in the case study.

7. Other instructions will be verbalized by the teacher in class
