(Grade 9- Carinosa)
1. Attendance is to be checked during the first 5 minutes every meeting (class checker)
*Late and absentees are to be recorded and must be submitted 5 minutes before the class
ends. Don’t wait for the teacher to remind you. Start practicing the sense of responsibility.
2. If in any case, you will be absent or late in the class, a PROPER excuse letter will bepresented
3. to the teacher by the following day stating your reason of absence/tardiness together with your
parent’s signature.
4. If the reason of your absence is with regards to your health, a medical certificate will be
attached with your excuse letter. However, if you did not see a physician and had just a bed
rest, let your parent’s sign your excuse letter.
5. If you missed an activity, a seatwork, assignment and most importantly, a quiz, do not wait for
the teacher to remind you of coping up what you missed. You should voluntarily ask
permission from the teacher to take whatever it is that you missed and wait for her to assign
date and time for you to take it. Failure to do so means that you are not interested to catch up
with the lesson and would also mean that you will receive zero in that particular activity/
seatwork/quiz/assignment.
6. Activities: Sometimes, an activity shall be assigned to you. The teacher always assign needed
materials 2 or 3 days prior to the activity. So, there will be no valid reasons for you to not bring
the materials assigned to you. INCOMPLETE or NO MATERIALS means ZERO ACTIVITY.
Borrowing or soliciting materials is strictly prohibited. There will be checking of materials
before the activity starts.
7. Submission Dates and Time: Let us value time very well. A well allotted submission date and
time will be given to you fairly for a requirement to be submitted. Late submission (submitting
beyond the time expected for the output to be handed over) will be given 75 no matter how
nicely done it is. Failure to submit will be automatically be given 0. Acceptance of late outputs
will only be 1 week after it’s deadline. Beyond that, it will be considered non-submission.
8. Quizzes and Periodic Tests: Strictly no cheating. Students caught doing such act shall
automatically receive 0. A letter informing the parent of such behaviour shall be issued. Such
behaviour will also be recorded on the anecdotal of the teacher.
9. Passing percentage per quiz: 75% of the perfect score will be considered as the passing grade
for each quiz. Failure to attain the percentage, the said student/s will be called for remedial
classes in which time will be determined by the teacher. Student/s will be given another set of
quiz for the said topic until he/she will be able to pass.
10. Compilation of Activities: All activities shall be compiled in a red expanded folder. Each
grading period, compiled activities shall be checked and collected by the teacher. Examples are
written works, like essays, reaction paper etc.. Lost activity means the student will make a new
one as a replacement of the original output. This will be one of the major basis of clearance
signing at the end of the school year.

the more if it’s a school property. In case of unforeseen circumstances that you have
damage a school property, you are responsible to replace the item/s.
12. Maintain cleanliness inside the classroom, please treat this as your home. Proper
disciplinary action should

nonsense talk) especially during activity proper, group works and above all during
discussion to avoid disturbance of fellow learners and to ensure harmonious flow of
discussion. Continuous observance of this action is equivalent to the following penalties:
a. First offense: Verbal Warning
b. Second offense: A Letter of Explanation is required with parent’s signature.
c. Third Offense: A Letter of Recommendation will be issued by the subject teacher
addressed to the adviser for proper discipline and will be noted to his/her anecdotal
record.
14. No using of gadgets (CELLPHONES, TAB, AND OTHER ELECTRONIC DEVICES
SUCH AS HEADPHONES during class hours. Offense for this will be subject to:
a. First Offense: Verbal Warning
b. Second Offense: A Letter of Explanation is required with parent’s signature.
c. Third Offense: Confiscation of gadgets. It will be returned only if the parent/guardian
will be the one to retrieve it.
15. Charging of gadgets in the classroom is strictly prohibited.
16. Romantic relationship is strictly prohibited.
