RENANTE SOBIACO PERDIDO, of legal age, Filipino, single and a resident
of Block 7, Lot 1, Phase 2, Jade Valley, Davao City, Philippines, after having
been duly sworn to under oath in accordance with law, do hereby depose and
say:

That WE personally know JANETH BAUTISTA TABUNO, being the

daughter of ROMEO TABUNO and MA. FATIMA TABUNO;

That WE know for a fact that she was born on November 17, 1978 at
Kapalong, Davao;

That in her Social Security System E-1 Personal Record bearing SSS
No.0920027626, her name is indicated as JANETH BATONGHINOG TABUNO:

That in her Certificate of Live Birth her name is indicated as JANETH

BAUTISTA TABUNO;

That the names, JANETH BATONGHINOG TABUNO and JANETH

BAUTISTA TABUNO, respectively refer to one and the same person;

That we are not related to JANETH BAUTISTA TABUNO either by affinity

or consanguinity;

That WE are executing this affidavit in order to attest to the truth of the
foregoing statements and to apprise the proper authorities thereof.
