Guidelines for Request for Revision in PCMA

For TE due to VO:


1. Contractual Data
2. NTP
3. Approved VO (Executive Summary)
4. Approved VO cover page (landscape)
5. Itemized Cost of Revision
6. Derivation of TE
7. Approved revised S-Curve

For TE due to Rainy/Unworkable Days:


1. Contractual Data
2. NTP
3. Signed WSO and WRO
or Monthly Suspension Report
4. Approved TE (Executive Summary)
5. Certification from PAGASA
6. Weather Chart
7. Pictures
8. Certified copy of Logbook
9. Approved revised S-Curve

For TE due to ROW problem:


1. Contractual Data
2. NTP
3. Signed WSO and WRO
4. Approved TE (Executive Summary)
5. Certification from mayor/barangay captain
or communication letter
6. Pictures showing ROW problem
7. Approved revised S-Curve

For TE due to Obstruction/Inaccessibility:


1. Contractual Data
2. NTP
3. Signed WSO and WRO
4. Approved TE (Executive Summary)
5. Pictures showing the affected site (before and after)
6. Communication letter
7. Straight line diagram/structure mapping (if any)
8. Approved revised S-Curve
For TE due to pending demolition/tree cutting permit:
1. Contractual Data
2. NTP
3. Signed WSO and WRO
4. Approved TE (Executive Summary)
5. Pictures showing the affected site
6. Communication letter
7. Copy of the permit
8. Approved revised S-Curve

REMINDERS:
- Observe proper documentation and check the completeness of documents
before submitting it at the Central Office to avoid any inconvenience

- Please indicate in the folder what particular revision/s is/are already


approved (if any), submitted and/or resubmitted
Ex.
TE # 1 = 60 cd already approved
TE # 2 = 30 cd already approved
TE # 3 = 30 cd Resubmission
VO # 1 = 20 cd Request

- For resubmitted request, properly label additional documents submitted


together with the evaluation sheet of BOC – PMD and return memo
previously attached by this office

- Do not attach this to the documents as this is just a guide for your requests
and may change anytime
