Dear Governor Remulla:

The Medical Assistance to Indigent Patient Program was tasked to assists our indigent and
poor patients seeking medical assistance in the LGU Hospitals in CaLaBaRZon.

In order to thresh out all issues and concerns about the program, the Department of Health
Regional Office 4-CaLaBaRZon will conduct a 3rd Quarterly Meeting on Medical
Assistance to Indigent Patients Program with MAP Focal Persons and Social
Workers of LGU Hospitals in CaLaBaRZon.

We would like to invite the MAP Focal Person and Social Worker of the following
Government hospitals to attend on August 30-31, 2018 at Eurotel Hotel Vivaldi, Cubao,
Quezon City.

1. General Emilio Aguinaldo Medical Hospital/Korea-Philippines Friendly Hospital


MAP Focal Persons: Ms. Vilma Dalusong
MAP Focal Persons: Ms. Marineth Villalobos
2. Kawit-Kalayaan District Hospital
MAP Focal Person: Ms. Irene Dimapilis
3. CARSIGMA District Hospital
MAP Focal Person: Ms. Congregacion Abasolo
4. Bacoor District Hospital
MAP Focal Person: Ms. Wilma Bayas
Social Worker: Ms. Hazel De Jesus

Board and Lodging shall be charged against LHSD-MAP (Medical Assistance Program)
fund, while travelling expenses shall be charged against their respective agencies, subject
to the availability of funds and usual accounting and auditing rules and regulations.
