TERMS & CONDITIONS

I. Training fees are collected on a Monthly Basis. All payment are to be made on the first week of the month. Fees are paid via Internet
Banking – Singapore Tennis Association, OCBC Bank (Current) A/C no.: 543-036990-001. Please indicate your child’s name & program
for reference.

II. Withdrawal from the RISING STARS TENNIS PROGRAM is STRICTLY on a Monthly Basis. Any withdrawal from the program can
be done through email – info@singtennis.org.sg.

III. Training sessions are outlined as such and subject to change by the Head Coach:

IV. The RISING STARS PROGRAM only supports children under 13 years old. The athlete shall attend and follow all training and
competition schedules as programmed.

V. The athlete’s obligations and responsibilities are the following:

1. Minimum monthly attendance of 75%, with the exception of travel for tournaments, injury accompanied by a medical report
or any other time off approved by the Head Coach.
2. Participating in a minimum of four (4) tournaments selected by the Head Coach within a 12-month period, the expenses of
which will be borne by the athlete.

VI. The Singapore Tennis Association (STA) reserves the rights to the following actions: the allocation of a trainee to a suitable class
according to the skill level of the trainee; any photographs or video or any form of media content taken at the facility to be used for
publicity or advertising; collect and use your personal data that you have filled out in the registration form for marketing and promotional
activities in the future.

Statement of Consent (To be completed by Parent/Guardian)

child’s/ward’s participation in the RISING STARS SCHEME. I have read and understood the above terms and conditions, and agree to abide by
them.
I hereby agree that I shall not hold the STA and its servants or agents responsible in any way for any injuries or loss of life or damage to property
howsoever arising out of or in the course of or in connection of the above program.
I further agree that I shall indemnify and keep the STA and its servants or agents indemnified against any claim or action which may be
instituted against STA or its servants or agents by any third party arising from my child’s/ward’s action or conduct in the course of or in
connection of the above program.
