AFFIDAVIT OF VERIFICATION OF RESIDENCY

I, BEVERLY ILUSTRISIMO-TUTOR, of legal age, Filipino citizen, married, and

residing at Bulacao, Cebu City, Cebu, Philippines, after having been duly sworn to in accordance
with law, do hereby depose and state THAT –

1. I am the legal counsel hired by Petitioner Abimelech Trinidad to handle his case
for Declaration of Nullity of Marriage against his spouse LENIE INTONG TRINIDAD.

2. I have personal knowledge about the residency of the Petitioner considering that
we are both active members of our church, Iglesia Filipina Independiente (IFI), with address at
Barangay Pari-an, Cebu City. I know for a fact that he is a resident of Cebu City as I have visited
their home at 46-E. St. Paul St., Barangay Hipodromo, Cebu City Cebu, during our Religious
gatherings and birthday occasions of his parents. He is married but is still living with his parents
in the aforesaid house especially so that his marriage with Lenie did not work out.

3. Being in the same church, we treat each other as families and friends. I know for
a fact that they used to live near our church at Barangay Pari-an, Cebu City, Cebu City but later
transferred to 46-E. St. Paul St., Barangay Hipodromo, Cebu City Cebu, Cebu, still within Cebu
City since 2010 up to present.

4. This Affidavit is executed to attest and certify the truth of the foregoing statement
in compliance with A.M. No.02-11-10-SC as recently amended by Supreme Court En Banc
Resolution dated October 2, 2018.

IN WITNESS WHEREOF, I have hereunto set my hand this 5th day of March, 2019 in
