CDA-OJT DEPLOYMENT EXEMPTIONS / OJT HOUR ACCREDITATION

1. FULL-TIME EMPLOYED STUDENTS. Students employed in CDA-relevant institutions/companies on a full-time


basis may file for exemption from performing OJT by submitting the following:
a. Complete name and address of institution/company and contact details of appropriate point person(s);
b. Duly signed Daily Time Record, which indicates that the student has performed at least 500 hours of
employment/service;
c. Job description issued by the Human Resource Office or Immediate Supervisor;
d. Copies of documents (e.g. research papers, projects, pictures, citations, certificates) to serve as: i) proofs
of involvement in workplace activities; and ii) proofs of accomplishments of the student as an employee;
and
e. A 5-Page Report citing the student’s insights on their work in the institution/company and its relevance to
the program, among others.

2. STUDENTS WITH SPECIAL NEEDS. Students with special needs who have been endorsed by the Center for
Counseling Services (CCS) for On-Campus Internship must coordinate with the CDA-OJT Coordinator and/or
Chairperson for documents processing.

3. EXTRACURRICULAR ACTIVITIES. Students who have previously participated in co-curricular/extracurricular


activities officially sanctioned by the CDA Program for OJT Hours Accreditation need only complete the remaining
number of hours upon completion of the said activities. Below is a table, which stipulates the equivalent of
participation in each co-curricular/extracurricular activity in OJT hours.

ORIENTATION PROGRAM STANDARDS

A term prior to taking up CDA-OJT, eligible students shall be required to undergo the CDA-OJT
Orientation, which aims to:

1. Provide an overview of the course objectives, structure, and requirements;


2. Inform incoming CDA-OJT students on the processes, requirements, standards, and
government and institutional policies that regulate On-The-Job Training.
3. Encourage students to raise questions, comments, or concerns pertaining to CDA-OJT;
4. Allow the CDA-OJT Coordinator to establish contact with incoming CDA-OJT students;
5. Ensure compliance with regulatory policies of government and institutional bodies.

What requirements are you expected to fulfill?

To request an endorsement, please follow the steps below:
a. Go to the CDA Office and ask for an “CDA Endorsement

b. Fill out the form with the necessary information. Please
make sure that the info you provide is up-to-date. You
can call/write to your prospective OJT sites to verify.

c. Submit the form to the secretary for processing. This
request normally takes 1-3 working days to complete.
d. Wait for your endorsement letters to be released.
e. Attach endorsement letter to your application.

What happens when you do not complete on time?
CONDITIONS FOR DEFERRED GRADES

Students who fail to meet the following conditions shall automatically be given a 9.9 (Deferred Grade):

1. Fulfill all Pre-OJT Requirements;


2. Complete required number of OJT hours before the deadline of submission of requirements
set by their respective CDA-OJT Adviser; and
3. Submit complete set of Final Requirements on or before the deadline of submission of
requirements set by their respective CDA-OJT Adviser.

Pursuant to the “Revised Policy on 9.9 Deferred Grade” (May 2016), students are given two (2) terms
to complete their pending requirement(s), and change their 9.9 (Deferred) to a grade that does not
exceed 2.0.

Students with CDA-OJT as their sole pending requirement to complete the CDA Program are required
to pay the Residency Fee.

CONDITIONS FOR FAILING THE COURSE

The following are grounds for receiving a grade of R in CDA-OJT:

1. Failure to comply with pending requirement(s) within the prescribed period;


2. Failure to attain a passing grade of 70% upon assessment of submitted requirements due to
unsatisfactory performance; and
3. Committing serious violation of the law and provisions in the Student Handbook (e.g. theft,
falsifying documents, perjury, etc.)

What is the Post-Training Self-Assessment?

POST-TRAINING SELF-ASSESSMENT GUIDELINES

The Post-Training Self-Assessment has been designed to streamline the processing of student
feedback in relation to training with their respective HTEs. Two different Self-Assessment forms
have been created for internship and extra-curricular activities. For a preview of the form,
please refer to pgs. 24-27.

What should be included in your Narrative Report?

NARRATIVE REPORT GUIDELINES

CDA-OJT Students need no longer submit a Narrative Report along with their other Final
Requirements effective T3 AY 2017-2018. Instead, they are to fill out the Post-Training Self-
Assessment Form found in pages stipulated in the previous segment. Do note, however, that
students qualified to apply for exemption from deployment (see pg. 8) are still required to
produce a Narrative Report, in addition to other required documentary evidence.

MEMORANDUM OF AGREEMENT

by and between

DE LA SALLE-COLLE OF SAINT BENILDE, a tertiary educational institution duly registered and organized under Philippine
laws, with principal address in the Philippines at 2544 Taft Avenue, Malate, Manila, represented in this MOA by MR. GARY G.
ADOR DIONISIO, Chairperson of the Consular and Diplomatic Affairs Program, co-signed by MS. CATHERINE D.
SAMANIEGO, Practicum Coordinator, hereafter referred to as “BENILDE”;

And

NAME OF HOST TRAINING ESTABLISHMENT, a duly registered and recognized CHOOSE OR INDICATE
TYPE/CLASSIFICATION (private company/ non-profit organization/ foreign chamber/ government office) existing under the
laws of the REPUBLIC OF THE PHILIPPINES, with address at Insert Address Here, represented herein by NAME OF
REPRESENTATIVE, its POSITION OF REPRESENTATIVE, and hereafter referred to as the “HOST TRAINING
ESTABLISHMENT”;

Recitals:

A. BENILDE and the HOST TRAINING ESTABLISHMENT promote mutual understanding in providing On-the-Job
Training opportunities to students of the Consular and Diplomatic Affairs Program (hereafter referred to as “CDA”).

B. Through this Memorandum of Agreement (MOA), both BENILDE and the HOST TRAINING ESTABLISHMENT
(collectively referred to as the “Parties”), desire to cooperate and collaborate with one another, to create experiential
learning opportunities for undergraduate students of CDA (hereafter referred to as STUDENT INTERNS), enhance
their understanding of program-relevant industries, and enrich their leadership and management insights through
hands-on activities and mentorship.

C. On these premises, BENILDE and the HOST TRAINING ESTABLISHMENT have agreed as follows:

I. STUDENT INTERNSHIP PROGRAM

This MOA seeks to establish and facilitate opportunities for structure learning and experiential study that will enhance
the academic leadership and management skills of CDA STUDENT INTERNS from BENILDE, through industry-
relevant training activities.

II. EFFECTIVITY AND DURATION OF MOA

2.1 The Parties intend to establish a partnership of cooperation and collaboration for an initial period of one (1) year,
effective from the signing of both parties of this MOA, and shall be deemed terminated upon expiration of said
period without need of any action, judicial or otherwise on the part of the Parties herein (the “Term”). This MOA
may be renewed for an additional period upon the written agreement of the parties under such terms and
conditions as may be acceptable to them, by prior written notice of thirty (30) days before its expiration.

2.2 Either Party may terminate this MOA prior to the specified date of expiration for any cause, by serving a written
notice of termination to the other parties at least seven (7) days prior to the intended date of termination.

2.3 Every academic year within the duration of this period, the HOST TRAINING ESTABLISHMENT shall host CDA
STUDENT INTERNS as trainees at its various facilities to support BENILDE’s Consular and Diplomatic Affairs
On-The-Job Training (hereafter referred to as CDA-OJT) for a maximum duration of Fourteen to Sixteen (14-16)
weeks per term.

2.4 The HOST TRAINING ESTABLISHMENT will also invite BENILDE to participate on its other programs and
activities that are relevant and suited to BENILDE Students.

III. ROLE OF BENILDE

3.1 Brief eligible CDA STUDENT INTERNS prior to deployment to HOST TRAINING ESTABLISHMENTS through
the following: a) Practicum Preparation Seminar Workshop (hereafter referred to as PRACPREP); and b) the
CDA-OJT Orientation (hereafter referred to as CDAO).

3.1.1 PRACPREP aims to:
a. Prepare eligible STUDENT INTERNS for transition from the academic to professional world
by conducting tests to help determine their strengths, weaknesses, and interests;
b. Offer valuable insight on preparing standard application requirements such as the Cover
Letter, Curriculum Vitae, among others; and
c. Provide helpful feedback on the strengths and weaknesses of eligible STUDENT INTERNS
through a job interview simulation.

3.1.2 On the other hand, CDAO is designed to:


a. Underline the course objectives, structure, processes, and requirements for eligible
incoming CDA-OJT students;
b. Establish direct communication channels between incoming CDA-OJT Students and the
CDA Practicum Coordinator.
c. Ensure compliance with regulatory policies of the government and other relevant entities.

3.2 Observance with HOST TRAINING ESTABLISHMENT’s policies. BENILDE’s CDA STUDENT INTERNS are
expected to observe the applicable policies of the HOST TRAINING ESTABLISHMENT throughout the duration
of the internship program. The school policies of BENILDE will continue to apply to students throughout the
program.

3.3 Coordination. The CDA Practicum Coordinator shall serve as the primary point of contact between BENILDE
and the HOST TRAINING ESTABLISHMENT. Both CDA STUDENT INTERNS and the HOST TRAINING
ESTABLISHMENT are to communicate concerns and other relevant information through the CDA Practicum
Coordinator.

3.4 Welfare and Safety. BENILDE, in partnership with the HOST TRAINING ESTABLISHMENT, shall help ensure
the welfare of Students by periodically gathering feedback from CDA STUDENT INTERNS; conducting
scheduled On-Site visits to the facilities of the HOST TRAINING ESTABLISHMENT; and exchanging relevant
information with the HOST TRAINING ESTABLISHMENT’s assigned representative.

IV. ROLE OF HOST TRAINING ESTABLISHMENT

4.1 Training Program. The HOST TRAINING ESTABLISHMENT will collaborate and cooperate with BENILDE in
developing training programs for STUDENT INTERNS. Training Programs must encourage the enhancement
of leadership, management, and communication skills, in line with BENILDE and CDA’s Objectives and Core
Values.

4.2 Mentorship. The HOST TRAINING ESTABLISHMENT shall designate a member to provide mentorship to CDA
STUDENT INTERNS in the form of an open, honest, and objective exchange of feedback; offering advice and
encouragement; and sharing best practices.

4.3 Coordination. The HOST TRAINING ESTABLISHMENT shall appoint a representative to serve as contact person
to exchange relevant information and feedback with BENILDE.

4.3 Welfare and Safety. The HOST TRAINING ESTABLISHMENT shall undertake measures necessary in ensuring
the welfare of CDA STUDENT INTERNS within their premises and during activities (i.e. lunch break, safe work
conditions and environment, work ethics, professionalism).

4.4 Deployments outside of Metro Manila. Should the HOST TRAINING ESTABLISHMENT deem it necessary to
deploy CDA STUDENT INTERNS to areas outside of Metro Manila for an activity or project, the HOST
TRAINING ESTABLISHMENT must notify the Practicum Coordinator at least five (5) days prior to scheduled
departure. Further, under these circumstances, food, transportation, and accommodation costs must be covered
by the HOST TRAINING INSTITUTION.

V. MUTUAL RESPONSIBILITIES OF THE PARTIES

BENILDE and the HOST TRAINING INSTITUTION shall:

5.1 Assign point persons for proper coordination, monitoring and evaluation of programs and activities.

5.2 Create a mechanism for consultations, monitoring and evaluation, with specific timeframes.

5.3 Carry out the respective protocols of both parties to ensure the safety and security of CDA STUDENT INTERNS.

5.4 Execute and deliver such further documents and instruments and may take such other actions as may be
reasonably appropriate or required by the Parties in order to carry out the intent and purposes of this MOA.

VI. GENERAL PROVISIONS

6.1 All Parties agree to hold the other free and harmless against any and all claims, damages, demands and/or
liabilities arising out of or in any way caused by or connected with the Parties’ own respective breach or violation
of the obligations, warranties, and terms and conditions, hereunder including claims from third Parties.

Every party acknowledges that the name and goodwill of other Parties will be associated with this MOA and will
thereby exercise of utmost good faith and observe due care and diligence in all its dealings, ensure continuous
compliance with all applicable laws, ordinances, codes, requirements, standard rules and regulations.

6.2 Execution. Each party shall review and approve this MOA, signed by a duly authorized representative(s). This
MOA may be amended only by the mutual agreements of the Parties, in writing.

6.3 Settlement of Disputes. In case of dispute, the Parties agree to exert all efforts to arrive at a mutually acceptable,
amicable resolution.

6.4 Relationship. Nothing in this MOA shall be construed to creating any employment, independent contractor, or
agency relationship between the Parties.

Executed in two (2) original copies, each party acknowledges having received one original copy.
