PETITION FOR COMM ISSION

AS NOTARY PUB LIC

PETITIONER, ATTY unto this Honorable Court, most respectfully

states that:

1. Petitioner is twenty-eight (28) years old, Filipino Citizen, single

and a resident of #51 JP Rizal Street cor. V.Giron Street, Brgy. 2,
Sta. Joaquina, Laoag City, Ilocos Norte, Philippines since 2013.
Petitioner’s date of birth is September 9, 1990. A machine copy
of her Certificate of Live Birth is attached hereto and made an
integral part hereof as Annex “A”.

2. Petitioner has satisfactorily completed the requirements of his

Juris Doctor degree and has successfully passed the 2018 Bar
Examinations and admitted to the practice of law in the
Philippines on June 20, 2019. A machine copy of her
Certificate of Bar Membership issued by the Office of the Bar
Confidant of the Supreme Court is hereto attached and made an
integral part hereof as Annex “B”;

3. Petitioner maintains a regular place of work at Perera Law Office

located at #62 JP Rizal Street, Brgy. 6, San Agustin, Laoag City,
Ilocos Norte, Philippines, where she may be served with
summons and other processes of this Honorable Court.

4. Petitioner is a member of the Philippine Bar in good standing
with Clearances from the Office of the Bar Confidant of the
Supreme Court, as well as from the National and Local
Chapters of the Integrated Bar of the Philippines. Copies
thereof are attached hereto and made an integral part hereof as
Annex “C”, “D” and “E”, respectively;

5. Petitioner is a registered member of the Integrated Bar of the

Philippines (IBP) with IBP No. 087333, and has paid all his
membership obligations, in both National and Local Chapters.
Copies of the receipts issued for this purpose are hereto
attached and made an integral part hereof as Annexes “F” and
“G”, respectively;

6. Petitioner has not been charged nor convicted in the first

instance of any crime involving Moral Turpitude with clearances
from the Municipal Trial Court of Laoag City, Regional Trial
Court, and National Bureau of Investigation. Copies of the said
clearances are attached hereto and made an integral part hereof
as Annex “H”, “I” and “J”, respectively;

7. Petitioner has also paid her Professional Tax and Community

Tax with the City Government of Laoag. Copies of petitioner’s
Professional Tax Receipt and Community Tax Certificate are
attached hereto and made an integral part hereof as Annexes
“K” and “L”, respectively;

8. Petitioner has duly paid all the fees necessary to the filing of this
Petition. A machine copy of the Receipt issued by the Office of
the Clerk of Court, Regional Trial Court is attached hereto and
made an integral part hereof as Annex “M”;

9. Petitioner possesses all the qualifications and none of the

disqualifications to be granted a Notarial Commission.

10. This is petitioner’s first time to file a Petition for Notarial

Commission and she has not yet applied nor has been
commissioned as Notary Public in any other jurisdiction;

11. In compliance to the requirements of the Honorable Court for

Notarial Commission, attached hereto and made an integral part
of this petition is a document containing petitioner’s (a) Three
(3) Passport-sized photographs with light background taken
within thirty (30) days of the application and (b) petitioner’s
signature at the bottom part of the said photographs, marked as
Annex “N” and series.

 PRAYER

WHEREFORE, premises considered, it is most respectfully prayed

of this Honorable Court that:

a. The Petition for Commission as Notary Public filed by the

undersigned be acted favorably; and

b. Petitioner be granted a Commission as Notary Public within the

territorial jurisdiction of Laoag City and Ilocos Norte.

Other reliefs that are just and equitable under the premises are
likewise prayed for.

authentic records;

4. I further declare that I have not commenced any action of

the same nature and involving the same issue/s nor one is
pending before any Regional Trial Court, the Court of
Appeals or the Supreme Court or any tribunal or agency of
the government, and that if there is any such action or
proceeding which is either pending or may have been
terminated, I undertake to report the status thereof, that if I
should learn that similar action has been filed or pending
before the Supreme Court, the Court of Appeals or any
tribunal or agency, I undertake to report the same before
this Honorable Court within five days from notice thereof.
