II. MATTERS INVESTIGATED:


02. The complaints of JACKIE LOU ABERIA y RIVADULLA, 22 years old, employee of mercury
Drugs Store and residing at #69 Zambales St. Barangay San Martin de Porres Cubao, Quezon
City, RONALD RYAN y BLANCAFLOR, 38 years old, married, jeepney driver and residing at
#05 Banahaw St. Barangay San Martin de Porres, Quezon City and PSMS ADRIAN PAOLO
JAGON y DELA CRUZ, 40 years old, married, assigned at Holding Personnel Accounting,
Camp Crame Quezon City and Residing at MB-15 BCDA Ususan, Taguig City for ROBBERY
CASE.

03. The liabilities of suspects RAFAEL y DELA CRUZ, 23 years old, singe, jobless and residing at
183-F 25th Avenue, Brgy. East Rembo, Makati City ( Presently confined at East Avenue
Medical Center, Quezon City.) and CHARLES y ULSUGAN, @ COCOY, 25 year old, single
jobless, and residing at BLK-162, Lot 19, Adalla Street, Brgy Rizal, Makati City (Under
Custody) for ROBBERY CASE

04. The liability of SPO2 ADRIAN PAOLO Y DELA CRUZ, for SERIOUS PHYSICAL INJURIES
(At large)
III. FACTS OF THE CASE:
04. Investigation disclosed that on or about 5:30 a.m. of July 20, 2003, suspect later on
identified as RAFAEL y DELA CRUZ, together with his unidentified cohort were both
armed with knife and handgun respectively boarded a passenger jeep with plate number PYH-
138 driven by RONALD RYAN y BLANCAFLOR along Arayat Cubao, Quezon City and once inside
said jeep, suspects abruptly declared a hold-up and, divested the personal belongings of Ms.
JACKIE LOU ABERIA y RIVADULLA such as one (1) shoulder bag color black worth Php 1,000.00
containing red wallet worth Php 2,000.00, cash money amounting to Php 3,500.00, BPI ATM
card, two (2) credit cards HSBC, Philamcare, Health Card, Company ID, and some important
documents, carted away as well the belongings of other passengers.

05. Suspects, immediately alighted from the said jeep after obtaining the stolen item, however,
SPO2 PSMS ADRIAN PAOLO JAGON y DELA CRUZ who has also a passenger of said jeep
pursued the fleeing suspects, thus when he identifies himself as a POLICEMAN, suspect
RAFAEL y DELA CRUZ suddenly pointed his handgun to the pursuing policeman SPO2 PSMS
ADRIAN PAOLO JAGON y DELA CRUZ was impelled to drew his service firearm and fired to
the direction of the suspect hitting him on his left leg. Thereafter, SPO2 PSMS ADRIAN
PAOLO JAGON y DELA CRUZ recovered from the possession of the suspect the said
HANDGUN. Suspect RAFAEL y DELA CRUZ, was brought to EAST AVENUE MEDICAL CENTER
for medical treatment while the unidentified companion of the suspect DELA CRUZ boarded
to a get away tricycle driven by another male suspect to elude arrest carting away with him
the loots.

06. Further investigation revealed that on the night of July 20, 2019, the undersigned received a
telephone call from suspect RAFAEL y DELA CRUZ live-in partner identified as RONALYN
LEVITEN y IBALLO and divulged that she will disclose the whereabouts of another suspect
identified as a certain CHARLES y ULSUGAN, thereafter, we sought the assistance of some
personnel of Precinct no. 8 of the Station 7 and Bantay Bayan members of Brgy. Depores
that resulted to the arrest of suspect CHARLES y ULSUGAN consequently he was positively
identified by complainant/ arresting officer DELA CRUZ.

07. SPO2 PSMS ADRIAN PAOLO JAGON y DELA CRUZ will be charged for SERIOUS PHYSICAL
INJURIES for the injuries he inflicted to suspect RAFAEL y DELA CRUZ.

08. Suspect RAFAEL y DELA CRUZ was presently confined at the East Avenue Medical Center
and this investigator apprised him of constitutional rights inside the Hospital and same
opted to remain silent, similarly, suspect CHARLES y ULSUGAN also apprised of his
constitutional rights but opted to remain silent.

09. PSMS ADRIAN PAOLO JAGON y DELA CRUZ was referred to the SOCO Crime laboratory to
undergo a PARAFFIN TEST, likewise, the recovered HANDGUN was also submitted in order to
determine whether the same was serviceable or not.

IV. DISPOSITION

10. This case will be referred to the City Prosecutors Office for proper disposition.
