Role Profile

Job Title Team Leader (Care)

Role Purpose To provide supervisory support to a team of Home Care Workers and support
the Care Services Manager to deliver high quality domiciliary service to
vulnerable people living in North Somerset.

Reporting to Care Services Manager

The role profile provides an outline of the strategic direction of CarePlus, together with an overview of the
duties envisaged to be carried out as part of the role. A person specification is also included within the
profile. The role profile will be used during one-to-one meetings and appraisals with your manager, to
review your performance on a regular basis.

MAIN DUTIES & RESPONSIBILITIES

1. Provide domiciliary care to customers to enable them to stay as independent as possible within
their own homes. This involves:

• Providing person-centred support to all customers


• Creating people-centred support plans and risk assessments with customers that identify and
meet their needs as discussed at initial meetings
• Maintaining confidentiality and professional boundaries across all aspects of care delivered
and ensuring records/support plans are accurate and managed sensitively
• Managing the risks associated with Lone Working
• Maintaining an awareness and understanding of other legislation relevant to working in the
Care sector
• To monitor, and report on service debts and take appropriate actions to ensure they are
minimised
• To implement appropriate administrative systems to ensure the continued smooth running of
the service

2. Monitoring staff workloads and the quality of support being delivered. This includes:

• Scheduling of the work for the team and ensuring they have a clear understanding of the
people-centred support plan.
• Monitoring workloads and the quality of support being delivered.
• Holding regular team meetings, regular supervisions and offering support and guidance as
necessary
• Collating data to contribute towards monthly performance indicators
• Supporting direct reports to manage risks associated with Lone Working e.g. ensuring risk
assessments, lone worker systems and procedures are adhered to
• Ensuring policies and procedures are followed by Home Care Workers
• Assessing the competency of care staff through Grey Matter system and observations.
3. Contribute to the development of the Service. This involves supporting the Care Services Manager
to:

• Carry out Service Promotion activity to raise the profile of CarePlus services
• Meet Quality Assessment Framework & CQC standards
• Continually assess the needs of all clients to ensure the service received meets their needs.
• Ensure safeguarding issues are reported promptly
• Response to complaints in a timely manner, seeking resolutions when possible.
• Continuously inputting evidence of service quality into the QuiqCare system.

4. Having responsibility for a duty phone dealing with the emergencies that may arise within your
team. . Please note there will be an allocated budget to provide relief from this responsibility
which will be the equivalent to two weeks per month to use at your own discretion

Line Management Responsibility

Care work is delivered from 7am to 11pm Monday to Sunday

Provide line management and supervision of a team of 10 to 15 Home Care Workers who will work
various different work patterns inclusive of early mornings, evening and weekends. Organise their
workload and provide guidance, training and good practice.

This responsibility includes:


• Organising and supervising the work undertaken by the team.
• Ensuring that individual performance targets are set and monitored.
• Addressing performance issues, dealing with discipline and absence management at an informal
level.
• Compiling monthly monitoring data in line with the key performance indicators.
• Participating in recruitment and setting up and implementing Induction Plans for new team
members
• Providing regular one-to-ones for direct reports
• Completing Annual Appraisals and identifying training and development requirements for direct
reports
• Developing team members e.g. through sharing of knowledge and providing advice on person
centred support
• Carrying out a range of supervisory tasks, examples include: completing Probationary
Assessments, Return to Work forms, approval of Annual, Leave

This role works closely with the Care Services Manager who will manage formal performance,
disciplinary and absence management issues.

Experience & Qualifications

• 3 GCSE Grade C or above or equivalent D

• Experience providing support in domiciliary care, support or health E
