1. That I am the mother of minor child CAEL SKYLER O. CHANG born on

July 1, 2014 at CENTRAL LUZON DOCTORS HOSPITAL (CLDH) Tarlac
City;
2. That I am giving my consent to my child to travel abroad to be accompanied
by my mother, Maria Zoraida C. Obar;

3. That my mother and I are sponsoring his trip and financially capable to
support his leisure / tour at HONGKONG;

4. That I guarantee that they will not violate any existing laws of their country
of destination (HONGKONG);

5. That I am executing this affidavit to attest to the truth of the foregoing

statements and for whatever legal intents and purposes it may serve.
