I agree to provide responsible care to include but not limited to proper nutrition,
sanitation, routine and necessary veterinary care
for__________________________ I am adopting. I agree the animal will reside
inside my home as a pet. I understand We Care For Paws Foundation is not
responsible for the accuracy of information received about the temperament,
habits, age or physical condition of animals available for adoption. We Care For
Paws Foundation makes every effort to be truthful. I have fulfilled my
responsibility to evaluate the above referenced animal myself before agreeing to
adopt it. I am in full agreement with the terms and conditions of this adoption
agreement. We Care For Paws Foundation will in no way be liable or responsible
for damages, accidents, injuries, or medical expense resulting from the above
referenced animal adopted into my home.

All medical and any other records on the above referenced animal being adopted
have been provided to me. It is my responsibility as the adopter to keep these
records, and to give this information to my veterinarian.

I UNDERSTAND AND AGREE TO RETURN THIS ANIMAL TO THE WE

CARE FOR PAWS FOUNDATION IF I CAN LONGER KEEP THE ANIMAL
FOR ANY REASON.

My signature on this adoption agreement represents my agreement with all

the conditions of this We Care For Paws Foundation Adoption Agreement, and
my commitment to fulfill my commitment as stipulated in this Adoption
Agreement. Should disagreements arise in the future, the parties agree to
abide by mediation.
