HUMAN RESOURCES DEPARTMENT

TO: JASOJASO, ALLAN (7330)

SUBJECT: NOTICE OF SUSPENSION

For: Serious Offense – Serious Misconduct

FROM: HR-INDUSTRIAL RELATIONS MANAGER

DATE: May 6, 2019

CC: HRD / MES/ NAFLU/ File

---------------------------------------------------------------------------------------------------------------------------------------------
This is in connection with the incident dated March 17, 2019. The report disclosed that on the date
mentioned, at around 0100H to 0400H, you were supposed to be on your work station but you were
nowhere to be found. Your superiors further reported that as they have visited your workstation to know
your whereabouts, they found no employee in your area. Hence, there is an irregularity regarding your
whereabouts from about 0100H to 0400H wherein even your superiors cannot justify your absence in your
supposed work station.

These acts of abandoning your place of work constitute Misconduct. Misconduct is the willful
transgression of some established and definite rules and policies. It implies wrongful intent and not mere
error of judgment. By leaving your work area without prior authorization, you did not merely disregard
company rules. You, in effect, failed to live up to the company’s reasonable expectations. No employer
may rationally be expected to continue in employment a person who lacks morals, respect and loyalty to
his employer, regard for his employer’s rules, and appreciation of the dignity and responsibility of his
office.

You were given a chance to explain through a memorandum issued to you on March 26, 2019. However,
you failed to justify in your explanation why you are not in your work station during above mentioned
time. This also the same during your interview with our Industrial Relations Superintendent Mr. Fernando
Ocampo last May 6, 2019. After a thorough evaluation and deliberation of the circumstances of your case,
Management found that there is a veracity in the charges against you.

For all the foregoing, the charge has been modified to “Abandonment of Post,” a violation of paragraph III
on Less Serious Offense of our revised company’s policies and procedures 135-3 and the Labor Code, as
amended. However, through compassion and understanding, the management would like to give you a
lesser penalty and another chance to rectify your actions. Hence, you are hereby sanctioned to receive a
five (5) days suspension. The days of your suspension will be scheduled and imposed in coordination with
the schedule to be given by your Superiors.

Be guided accordingly.
