Re: SUSPENSION
Per administrative policy, an accumulated number of 180 minutes and five (5) days or more being late for more
than 10 minutes instances in a month without justifiable reason shall constitute an offense of “tardiness”.
Under the policy, the offense of Tardiness is treated with the following disciplinary measures:
The record shows that you violated administrative policy on TARDINESS on Dec 10, 11, 12, 13, 19, 20, 21, 28, 2018
– One offense - at least 180 mins which appears to be your second offense respectively. You were given a chance
to explain through a memorandum, issued to you on January 30, 2019. However, we did not receive your
explanation.
In view of the foregoing, you are hereby suspended for five (5) days effective, ________________________. You
are further warned that repetition of the same or similar offense will be dealt with stiffer penalty.
