PROVISION OF EQUIPMENT SUPPORT SERVICES

SCOPE OF WORK
1.0 DESCRIPTION OF SERVICES TO BE PERFORMED

1.1 Contractor shall provide equipment that is acceptable by Company, with competent
operator and mechanic, on call-out basis, to support Company’s operations. The
equipment to be provided by Contractor shall include, but not limited to the following
as specified in a Service Order duly executed by Company and Contractor:
1.1.1 250-Ton Manitowoc 999 Crawler Crane Series 3
1.1.2 220-Ton 1400 Crawler crane
1.1.3 120-Ton RT 130 Grove Mobile crane
1.1.4 100 -Ton Manitowoc 10,000 Crawler Crane with operator and mechanic.
1.1.5 80-Ton RT Grove Mobile Crane
1.1.6 60-Ton Grove Mobile crane
1.1.7 30-Ton Trailer truck
1.1.8 Man-Lift JLG Ultra-boom
1.1.9 3.5 Ton Telescopic forklift
1.1.10 3.5 Ton Regular forklift
1.1.11 Coaster Bus with driver and mechanic
1.2 Contractor shall provide any or all of the following equipment operator services
that is acceptable by Company, on call-out basis, to support Company’s operations.
The equipment operator to be provided by Contractor shall include, but not limited to
the following as specified in a Service Order duly executed by Company and
Contractor:
1.2.1 Equipment Operator for a 250-Ton Manitowoc 999 Crawler Crane Series 3
1.2.2 Equipment Operator for a 220-Ton 1400 Crawler Crane
1.2.3 Equipment Operator for a 120-Ton RT 130 Grove Mobile Crane, class A.
1.2.4 Equipment Operator for a 80-Ton RT Grove Mobile Crane, class A.
1.2.5 Equipment Operator for a 60-Ton Grove Mobile Crane, class A.
1.2.6 Equipment Operator for a 30-Trailer truck, class A.
1.2.7 Equipment Operator for a Man-Lift JLG Ultra-boom.
1.2.8 Equipment Operator for a 3.5 Ton Telescopic forklift.
1.2.9 Equipment Operator for a 3.5 Ton Regular forklift.
1.3 Contractor shall provide any or all of the following rigger services that is acceptable
by Company, on call-out basis, to support Company’s operations. The riggers to be
provided by Contractor shall include, but not limited to the following as specified in a
Service Order duly executed by Company and Contractor:
1.3.1 Rigger; class 3.
1.3.2 Rigger; class 4.
1.4 Contractor shall be responsible for the provision of equipment with full complement
of competent crew (e.g. Operators and Mechanics) to enable 24-hour operations
throughout the duration of a call-out.
1.5 Contractor shall be responsible for safe operations, crewing, and maintenance of own
equipment, at own costs, without recourse to Company.
1.6 Company shall make request for equipment service using Service Order duly
executed by Parties (Company and Contractor).
1.7 Contractor shall receive compensation for period of satisfactory service(s) only –
prorated for fraction of a day where applicable.
1.8 Company will record period of service using Activity Log Sheet prepared by
Company.
1.9 Quality of Service will be determined if satisfactory or not, by Company
Representative.
1.10 Any equipment provided for service under this contract or a Service Order with a year
of manufacture prior to 2010 may be considered for use only if accepted by
Company’s Operations Representative. Nevertheless, all equipment must be cleared
by Safety, Compliance, and Operational representatives following pre-mobilization
inspection conducted by Company.
1.11 For all Equipment provided for service under the contract, Contractor shall provide
competent and qualified operators and mechanics adequate to provide 24 hours daily
service throughout the duration of a Service Order. Such operator and mechanic
positions shall be occupied by rotational prequalified persons.
1.12 Provision of other equipment as specified in a Service Order duly executed by
Company and Contractor.
1.13 Services provided will be for twenty-four (24) hours availability while on call-out.
1.14 Contractor shall provide all lubricants required for safe and efficient running of
equipment.
1.15 Delivery and Redelivery location for the equipment and personnel shall be Chevron
Dock, Warri or as specified in applicable Service Order.
1.16 Contractor shall receive compensation for the number of day(s) Service was provided
under a Service Order, as specified in relevant activity log sheet duly endorsed by
Company’s Representative.
1.17 Contractor shall be responsible for the removal of own equipment and personnel from
Company’s area of operation at the expiration of a Service Order.
1.18 Contractor shall ensure that equipment provided to perform services under this
Contract are compliant with Company’s Health, Environment & Safety, Marine
Safety, Reliability & Efficiency standards, and applicable statutory regulations.
1.19 Only contractors whose equipment are found satisfactory following site visit will be
considered to provide service at any time.
1.20 Equipment Day Rates for all cranes and forklifts are inclusive of mechanics and Class
A certified equipment operator(s).
1.21 Operator Certification / Qualification: All equipment operators are to have valid
certifications as detailed below:
1.21.1 Crane operators - Certification from Company Warri Training Center.
1.21.2 Forklift operator - Certification from Company Warri Training Center.
1.21.3 Trailer Driver National Driving License - Class F.
1.21.4 Costs of all Chevron required Operator Certifications and Recertification (at
Warri Training Center and elsewhere) are to be borne by Company. All other
associated costs by Contractor (accommodation and meals) are to be borne by
Contractor.
1.22 Contractor shall not be paid compensation for an available equipment if the equipment
operator does not have all require Certifications.
1.23 Pre-mobilization inspection requirement by Company shall include, but not limited
to:
1.23.1 Scan copy of Contractor/Vendor completed SOP 504 Pre-mob Inspection
Form (Pre-mobilization inspection is conducted by Company and the form is
made available to Contractor when requested).
1.23.2 Scan copy of current annual inspection reports and documentations of
applicable equipment.
1.23.3 Scan copy of current annual load test reports and documentation.
1.23.4 Scan copy of current MPI or NDT inspection reports on hooks and
documentation.
1.23.5 Scan copy of Operator(s) certification(s).
1.24 Maintenance:
1.24.1 Preventive Maintenance Schedule for equipment (Quarterly; Annual PM)
shall be communicated by Contractor to Company monthly.
1.24.2 Allowable scheduled preventive maintenance time is 24 hours per calendar
month. No carry-over/accumulation of maintenance period.
1.24.3 Equipment will be put off-hire by Company for the period of unscheduled
downtime.
