This is to CERTIFY that AQUILINA CERNAL PARLOCHA, 99 years of age, is a resident

of Sitio Paraiso, Brgy. Refugio, Calatrava, Negros Occidental.

I visited AQUILINA CERNAL PARLOCHA in her home last September 10, 2019. To my
knowledge Aquilina is still living but she has a hearing difficulty. Moreover, she cannot walk
alone and needs assistance in doing so.

This Certification is issued upon the request of her grandson named RYANN CESAR
PARLOCHA ALFAR for whatever legal purposes it may serve him best.
