1. Reference: Memo from IG, IAS dated January 28, 2019 with subject same
as above.

2. Above reference pertains to the ongoing validation and auditing of all

Homicide Cases Under Investigation (HCUI) Other Causes of Death such as
(reckless imprudence, vehicular accident, drowning, hanging, electrocution, struck by
lightning, etc.)from July 2016 up to June 2018.

3. In this regard, you are hereby directed to submit to this Office NLT
10:00AM February 1, 2019the required data for consolidation and subsequent
submission to RIAS 9.

4. Attached herewith is the sample format for your reference.
