1 entry procedure? Is the procedure acceptable and comply with GL Confined 2 Space Entry Standard? Do you know the potential hazards of this confined space 3 entry? 4 Was the media stored known to you? 5 Any work in the vicinity known to you for assessment? Has the space been isolated from other systems and 6 equipment? Confined space atmosphere tested by client competent 7 person? Is the test equipment used, calibrated with valid 8 certificate? 9 Is oxygen level between 20.8% (+/-0.2%)? The level of flammable gas is less than <1% of its lower 10 explosive limits LEL? 11 Has the space been adequately ventilated before entry? 12 Will ventilation be continued during entry? 13 Will the atmosphere be monitored while work is going on? 14 Are you carrying your personal monitoring device? Is the air intake located in an area free of combustible 15 dusts, vapours and toxic substances/discharges? Is safe means of access/egress & lighting provided and 16 identified? Is another person accompanying you into the confined 17 space? Will there be a standby person on the entrance in constant 18 visual or auditory communication with the person on the inside? Has the standby team/person been trained for 19 emergencies? 20 Is rescue plan and equipment available on site? Has a confined space entry permit or equivalent been 21 approved and issued? Have you physically looked at the approved permit or 22 equivalent? 23 Is the permit within its validity? 24 Are you satisfied with the approved permit?
