1. What: Implementation of Search Warrant (Violation of RA 10591)

2. Where: Purok 3, Barangay San Lorenzo, Masinloc Zambales

3. When: Time implementation: at about 4:00 PM of September 17, 2019

Time of Arrest: 04:05 PM of September 17, 2019

4. Who: Suspect- Arnel Gabriel y Ubaldo, (Watch Listed), male, 29 years old, married Elementary
Graduate and a resident o Purok 3, Barangay San Lorenzo, Masinloc Zambales

5. How: Joint elements Masinloc Municipal Police Station led by the undersigned, CIDGPFU-
ZAMBALES, 1st PMFC and PNP-Maritime Zambales, implemented Search Warrant No. 059-19
issued by Hon. Judge Emmanuel A. Silva of RTC Branch 4 of Mariveles, Bataan, dated September
17, 2019 for Violation of RA 10591 (Firearms and Ammunitions Regulation Act).

Subject person was immediately placed under arrest upon confiscation of the following:

1. One (1) pc of Caliber 38 revolver with Serial Number 18357 marked with initials “NJG-P1,
MMV-P1”.
2. Six (6) pcs live ammunitions for Caliber 38 revolver marked with initials “NJG-P2, MMV-P2”
to “NJG-P7, MMV-P7”
3. Three (3) pcs small heat sealed plastic sachets containing suspected Methamphetamine
Hydrochloride (Shabu) marked with “NJG-P8” to “NJG-P10” and “MMV-P8 to MMV-P10”
4. Ten (10) pcs live ammunitions for Caliber 45 marked with initials “NJG-P11, MMV-P11” to
NJG-P20, MMV-P20”
5. One (1) pc open small transparent plastic sachet containing residue of suspected
Methamphetamine Hydrochloride (Shabu) marked with “NJG-P21” and “MMV-P21”
6. Two (2) pcs aluminum foils (runway) marked with “NJG-P22’ to “NJG-P23” and “MMV-P22”
to “MMV-P23”
7. Two (2) pcs rolled long aluminum foils (tooter) marked with “NJG-P24’ to “NJG-P25” and
“MMV-P24” to “MMV-P25”
8. One (1) pc rolled short aluminum foil (needle for improvised burner) marked with “NJG-P26”
and “MMV-P26”
9. Two (2) open lighters (Green and Yellow) marked with “NJG-P27’ to “NJG-P28” and “MMV-
P27” to “MMV-P28
10. One (1) pc small stick marked with “NJG-P29” and “MMV-P29”
11. One (1) pair scissor marked with “NJG-P30” and “MMV-P30”
12. One (1) pc perfume pouch with label “Afficionado” marked with “NJG-P31” and “MMV-P31”
13. One (1) pc used news paper (Gun Wrapper) marked with “NJG-P32” and “MMV-P32”
14. One (1) pc used plastic with label “apple” marked with “NJG-P33” and “MMV-P33”

6. Why/Motive: Implementation of Search Warrant

7. Action Taken: Suspect was temporarily held to this office for proper disposition and case of Violation
of RA 10591 (Comprehensive Firearms and Regulation Act) and Sec 11 and 12 of R.A 9165
(Comprehensive Dangerous Drug Act) are being prepared for filling against the arrested suspect
before the Office of Provincial Prosecution.
