Dear Ms Webster,

Re: Alison Cooper, D.O.B.:14/06/2002

This is to refer Alison, a 17-year-old girl, into your care for further support and assessment due to
supposed possible grief-related and other psychological issues.

Alison is a 5th-year student, (who lost her father 18th months ago) and she lives with her mother
who is a full-time bank manager. Having such a demanding job, Alison and her siblings are often
taken care of by their grandmother who lives near the school. Unfortunately, her father passed away
(mention this first) in a motor accident 18 months ago, and since then, she has started to be more
absent from school due to medical problems like headaches and stomach aches or because her
eczema got worse making her uncomfortable (due to medical problems such as various body aches,
nausea and eczema outbreaks/flare ups). In these situations, her grandmother was called to pick
her up.

Regarding Alison’s social background, she started school well, but since the 3rd Grade she has had
problems concentrating, avoiding class activities and not wanting to participate in school sports and,
her quality of schoolwork has declined. Moreover, she has a few friends, and her classmates often
make fun of her weight and eczema problems. Therefore, her mother was contacted, and she said
that Alison is withdrawn and cries at home because she misses her father, who she was very
attached to, and often finds comfort in unhealthy foods.

In the view of the above, it would be greatly appreciated if you could comfort and support Alison
regarding her situation. If you require further information, please do not hesitate to contact me.

Yours sincerely,

Registered Nurse
