Madam,

We are pleased to submit our formal quotation for the supply of labor, tools/equipment and
supervision necessary for the satisfactory completion of the above project under the following terms and
conditions:

SCOPE OF WORK:
1. Mobilization/Demobilization.
2. Documentation of existing structure for future claims and reference.
3. Preparation of concrete slab and locate hairline cracks.
4. Application of Concrete Epoxy Low Viscosity on chipped cracks to help on rebinding concrete
topping of slab.
5. Provide shoring jack or temporary support at middle of slab to prevent deflection on slab.
6. Slope correct existing concrete topping to elevate flooring and prevent water ponding.
7. Application of Cementitious waterproofing on exposed concrete surface.
8. Plastering of parapet walls and firewalls to stop water leakage.
9. Canting of corner to wall termination.
10. Application of 5 coats waterproofing of cementitious waterproofing with matting.
11. Cleaning and retouch works.

ESTIMATED COST OF LABOR AND MATERIALS ---------------------------- PhP 177,500.00

TERMS OF PAYMENT:
30% Down payment, balance thru progress billing

supervision necessary for the satisfactory completion of the above project under the following terms and
conditions:

SCOPE OF WORK:
1. Mobilization/Demobilization.
2. Documentation of existing interior structure.
3. Concreting of countertop slab 1.50m – 1.80m includes reinforce concrete to anchor on walls.
4. Installation of granite slab.
5. Provide drain for washing machine and supply of water line using PPR pipes.
6. Installation of power outlet for washing machine from panel board.
7. Fabrication of cabinet for kitchen countertop.
8. Installation of Owner supplied kitchen sink complete with accessories.
9. Cleaning and retouch works.
