RE: Claim # 27171425891200

Misdiagnosed

To Whom It May Concern:

I visited my doctor numerous times to express my concerns which they could not help me with. They did
multiple tests and did not identify the simple medical issue that I brought to their attention. Based on
the diagnoses of Seasons of Life OBGYN, I saw an expert in nerve damage. That doctor did a physical
evaluation and proposed a treatment plan. He devised a treatment plan based on nerve damage. This
doctor was out of network. I sought another doctor with similar specialty background. This doctor did
appropriate cultures and tests to diagnose me with a much typical condition than my previous office
could not find. They prescribed me multiple doses of antibiotics. The first two doses did not work. The
third dose seems to be improving my condition, however, I must take this antibiotic and antifungal
medication for approximately 6 months. As of now, I am recovering from both the infection, and yeast
infection. I am dissatisfied with the in-network physician’s office Seasons of Life OBGYN Dr. Emily Ciesk.
If she would have done an extensive panel like Dr. Dapuzzo did, my symptoms could have been
alleviated months ago. I could have saved thousands of dollars. I asked multiple times to have hormone
levels checked. I was told “NO” They prescribed me Estradiol without checking my levels. I am lodging a
formal complaint against Seasons of Life, OBGYN.

Sincerely,

Tami Herman
