AFFIDAVIT OF TEAM LEADER

I, WILLIAM S. SAVELLA, of legal age, Filipino and the Provincial
Environment and Natural Resources Officer of Isabela, after having been duly sworn
to in accordance with law do hereby depose and say to wit;

1. That on March 3, 2014, I was informed by the Department of Environment and

Natural Resources, Regional Executive Director (DENR-RED), that the said
office received reports from the DENR, Office of the Secretary, about the
rampant sale of common hard wood by several establishments located at Ilagan
City and Gamu, Isabela, without any valid lumber dealer permit to sell/possess
lumber from the DENR and then he instructed me to verify the veracity of the
said report which he received and directed me to conduct a test-buy operation of
common hardwood at the various hardware located in the said places which
were not issued Lumber Dealer permit.

2. After receiving the said instruction from the Regional Executive Director of the
DENR, I verified from the PENRO, records section, Ilagan, Isabela, the list of
establishments/Hardware which were issued Lumber Dealer Permit particularly
those that are operating in Gamu and Ilagan City, Isabela.

3. On March 4, 2014 at around 7:00 am, I immediately formed a team to conduct

surveillance and test-buy operation at Gamu, Isabela in order to determine
whether there are establishments which were not registered as lumber dealer.

4. Thereafter, on the same day at around 8:00 am, while our team was about to
proceed to Gamu, Isabela to conduct surveillance and test-buy, JERRY
CASTRO, Forest Ranger of the PENR Office, Ilagan City, Isabela, intercepted a
tricycle with plate number QH- 4729, color red, loaded with a lumber and he
invited them at PENRO, Ilagan, Isabela. Upon inquiry, the driver alleged that they
purchased the said lumber from HBA HARDWARE and he exhibited to us the
Official Receipt No. 1225 dated March 4, 2014 issued by HBA HARDWARE,
located at Upi, Gamu, Isabela. Copy of the Official Receipt anf the spot report is
hereto attached as annexes A and B.

5. After verification from our records in the PENRO,Ilagan City, Isabela, HBA
HARDWARE was not issued a Lumber Dealer Permit. Thus, I immediately
dispatched MAMERTO C. QUESADA and SHIRLEY S. CABANG, among the
members of our team, to proceed to Upi, Gamu, Isabela and then to inquire at
HBA Hardware whether it has stock of lumber and to canvass its price.

6. After a while, MAMERTO C. QUESADA and SHIRLEY CABANG returned to our

designated meeting place at the PENRO, Ilagan City, Isabela and informed me
that that they were able to canvass 1,600 board feet of common hard wood and
that the said establishment showed to them their available stocks of lumber and
one of the attendant therein provided with them a price quotation.

7. I am executing this affidavit for the purpose of securing a search warrant against
HBA Hardware authorizing us to conduct a search on the premises of the said
establishment which probably committed violation of PD 705 and other related
forestry laws, rules and regulations.
