AFFIDAVIT OF NON-FILING OF INCOME TAX RETURN

We, Jier Anthony S. Perez and Michelle M. Perez, both of legal ages, Filipino
citizens, and residents of Guiwan San Bernardino Bridge, Zamboanga, Philippines,
after being sworn to in accordance with law, depose and state that:
1. We are the parents of Francis Daniell M. Perez;

2. Francis Daniell M. Perez is under the process of enrolling for Grade 7 at

Zamboanga Sto. Niño Village School, Putik, Zamboanga City;

3. To avail of the free education program of the school, one of the

requirements is a proof of the Bureau of Internal Revenue (BIR) validated
Income Tax Return of parents;

4. We did not file our Income Tax Return at the BIR because we have no
income and are currently unemployed;

5. We are executing this affidavit to attest to the veracity of the foregoing

facts, in support of our son’s application for free education and for
whatever purposes this may serve.
