Dear Mrs Wilkins,

RE: Mrs Aisha Amari, 75years old

I am writing to refer Mrs Amari into your care, who needed medical care for her swollen
ankle.

Regarding Mrs Amari’s medical history, she has type 2 diabetes which is kept under control
by a low cholesterol diet and Diabex medication. Additionally, she needs to take Beta
blocker for her three-year-old hypertension.

Because of her left foot being turned out on ambulation, she developed a week ankle which
made her walking a difficult task. During her stay at the clinic, she prescribed to take
Calcium channel blockers and a deep heat cream for her ankle pain.

It is important to note that she can speak only Arabic language, but his son can provide any
information required.

Would you kindly monitor her ankle progress and that she will be assisted with personal care
and some house chores around four hours two days per week?
